【摘要】 目的:探讨维持性血液透析的尿毒症患者血尿酸水平、血脂水平与血压变化的关系及控制患者血尿酸和血脂对血液透析患者血压的影响。方法:选择103例维持性血液透析的尿毒症患者,根据血尿酸、血脂水平分为四组:高尿酸、血脂组(A组),高尿酸组(B组),高血脂组(C组),正常尿酸、血脂组(D组)。对比四组患者的治疗前及治疗18个月后s UA、TCH、TG)、LDL、HDL及血清ALB、Scr、C反应蛋白和凝血功能。结果:四组患者治疗前的s UA、TCH、TG、LDL、HDL对比差异具有统计学意义(P>0.05),治疗后其中A组和B组的s UA与治疗前对比显著下降(P<0.05),A组和C组TCH、TG、LDL和HDL与治疗前对比显著下降(P<0.05),治疗后的四组之间的CRP值、PT值和APTT值对比差异无统计学意义(P>0.05),其中A、B、C组之间的CRP值较治疗前显著下降(P<0.05)。A、B、C三组的收缩压和舒张压水平均较治疗前显著下降(P<0.05),且显著低于D组(P<0.05),收缩压和舒张压的变化与s UA、TCH、TG和LDL水平的变化均显著呈正相关(P<0.05),而收缩压和舒张压的变化与HDL水平变化呈均显著呈负相关(P<0.05)。结论:维持性血液透析的尿毒症患者血尿酸水平、血脂水平与血压变化的密切相关,积极控制血尿酸和血脂水平有助于维持性血液透析患者高血压的防治。更多还原

【Abstract】 Objective:To investigate the relationship between serum uric acid level,blood lipid level and bloodpressure changes and the effects of serum uric acid and blood lipid on hemodialysis patients with hemodialysis. Methods:103 patients with maintenance hemodialysis were divided into four groups:hyperuricemia and hyperlipidemiagroup(group A),high uric acid group(group B),hyperlipidemia group(group C),normal uric acid,blood lipidgroup(group D). TCH,TG,LDL,HDL,serum ALB and Scr,C-reactive protein and coagulation function were mea-sured before and after 18 months of treatment. Results:The s UA,TCH,TG,LDL and HDL of the four groups were sta-tistically significant before treatment(P>0.05). After treatment,s UA in group A and B was significantly lower than thatbefore treatment(P<0.05).The levels of TCH,TG,LDL and HDL in group A and C were significantly lower than thosebefore treatment(P<0.05). There was no significant difference in CRP,PT and APTT between the four groups aftertreatment(P>0.05),And the CRP between group A,B and C was significantly lower than that before treatment(P<0.05). The systolic and diastolic blood pressure of group A,B and C were significantly lower than those in group D(P<0.05),and significantly lower than that in group D(P<0.05).The change of systolic and diastolic blood pressure waspositively correlated with the changes of s UA,TCH,TG and LDL(P<0.05),while the changes of systolic and diastolicblood pressure were negatively correlated with the change of HDL(P<0.05). Conclusion:Serum uric acid level andblood lipid level of uremic patients in maintenance hemodialysis are closely related to the level of Blood pressure changes,and active control of serum uric acid and blood lipid levels can help prevent the prevention and treatment of Blood pressure changes in maintenance hemodialysis patients.更多还原
