【摘要】 目的分析快速尿素酶试验（rapid urease test,RUT）、¹³C-尿素呼气试验(carbon-13-urea breath test,¹³C-UBT)在幽门螺杆菌（Helicobacter pylori,HP）检测中的临床应用价值。方法对2015年5月-2016年5月佛山市第一人民医院收治的180例因上腹部不适前来就诊的患者进行观察,先后采用RUT、¹³C-UBT对HP进行检测,观察两种检测方法对HP的检出情况。结果 （1）180例患者HP阳性80例,阳性率为44.44%;阴性100例,阴性率为55.56%。（2）¹³C-UBT检出阳性56例,阳性率为70.00%;阴性88例,阴性率为88.00%。RUT检出阳性55例,阳性率为68.75%;阴性90例,阳性率为90.00%。两种检查方法间差异无统计学意义（P>0.05）。（3）¹³C-UBT对HP检测的特异性及敏感性均优于RUT,差异有统计学意义（P<0.05）。结论 RUT、¹³C-UBT对HP阳性检出率均较高,¹³C-UBT属于无创操作,特异性及敏感性明显优于RUT,适用于儿童、老人检查,但是对仪器要求高,具有放射性。RUT操作简单,检测时间短,属于有创检查,应根据患者病情及实际情况选择最佳检查方法。更多还原

【Abstract】 Objective To evaluate the clinical value of rapid urease test（RUT） and ¹³C-urea breath test(¹³C-UBT) in detection of Helicobacter pylori（HP）. Methods From May 2015 to May 2016 in The First People ’s Hospital of Foshan, 180 patients with upper abdominal discomfort were enrolled in the study. RUT and ¹³C-UBT were used to detect HP. The detection rates by the two methods were compared. Results The 80 cases showed HP positive and the positive rate was 44.44%; 100 cases showed negative results and the negative rate was 55.56%. 70 cases were detected positive by ¹³C-UBT and the positive rate was 70.00%; 88 cases showed negative results and the negative rate was 88.00%. 55 cases was detected positive by RUT and the positive rate was 68.75%; 90 cases showed negative results and the positive rate was 90.00%. There were no significant differences between the two methods（P>0.05）. The specificity and sensitivity of ¹³C-UBT to HP were superior to RUT, with significant differences（P<0.05）. Conclusion RUT and ¹³C-UBT both have high detection rate of HP. ¹³C-UBT is noninvasive, and the specificity and sensitivity are better than those of RUT and it is suitable for children and elderly people, but it is high in radioactive. RUT operation is simple, and the detection time is short. The best check method should be chosen based on the patient’s actual situation.更多还原
