【摘要】 目的探讨核磁共振联合血清生长分化因子(GDF3)、细胞白蛋白19片段(CYFRA211)、癌胚抗原(CEA)在乳腺癌诊断中的价值。方法选取63例乳腺病变患者作为研究对象,患者均采用核磁共振(MRI)检测,血清GDF3、CYFRA211、CEA测定采用酶联免疫法,观察患者手术病理诊断结果。以手术病理诊断结果作为参考,对比MRI诊断、血清诊断以及二者联合诊断效能以及受试者工作特征(ROC)曲线下面积(AUC)。结果 MRI检测乳腺癌患者诊断符合率91.18%,灵敏度91.30%,特异度90.91%,误诊率8.33%,漏诊率8.70%。血清GDF3、CYFRA211、CEA诊断符合率85.29%,灵敏度84.78%,特异度86.36%,误诊率15.22%,漏诊率13.64%。联合检测诊断符合率97.06%,灵敏度95.65%,特异度100.00%,漏诊率4.35%。联合诊断符合率、敏感度以及AUC值均高于单独检测,差异有统计学意义(P<0.05),而特异度无显著差异(P>0.05)。结论乳腺癌患者采用MRI联合血清GDF3、CYFRA211、CEA检测的诊断效果较好,对于乳腺癌的临床诊断具有一定的参考价值。更多还原

【Abstract】 Objective To investigate the value of magnetic resonance imaging combined with serum growth differentiation factor(GDF3),cell albumin 19 fragment(CYFRA211) and carcinoembryonic antigen(CEA) in the diagnosis of breast cancer.Methods 63 breast lesions were selected as the research object,all patients were examined by nuclear magnetic resonance(MRI),Serum GDF3,CYFRA211 and CEA were measured by enzyme linked immunosorbent assay(ELISA),the surgical pathologic results acted as reference,comparison of MRI diagnosis,serological diagnosis,combined diagnostic efficacy and receiver operating characteristic(ROC) area under the curve(AUC) value were compared.Results The diagnostic accuracy rate of MRI in breast cancer patients was 91.18%,the sensitivity of 91.30%,the specificity of 90.91%,the misdiagnosis rate was 8.33%,and the missed diagnosis rate was about 8.70%.The diagnostic accuracy rate of serum GDF3,CYFRA211 and CEA was 85.29%,the sensitivity was 84.78%,the specificity was 86.36%,the misdiagnosis rate was 15.22%,and the missed diagnosis rate was 13.64%.The diagnostic accuracy rate of combined detection was 97.06%,the sensitivity was 95.65%,the specificity was 100.00%,and the missed diagnosis rate was 4.35%.The diagnostic accuracy,sensitivity,and AUC value of the ROC curve were higher than those of each single test,the difference was significant(P<0.05),and there was no significant difference in specificity(P>0.05).Conclusion Breast cancer patients with MRI diagnosis effect is good,and diagnostic effect was significantly improved when combined with serum GDF3,CYFRA211,CEA diagnostic,which has certain value for clinical detection of breast cancer.更多还原
