【摘要】 目的探讨心电图对判断变异型心绞痛(variant angina pectoris,VAP)患者痉挛靶血管部位的临床价值,为诊治策略的选择提供依据。方法 VAP患者160例,根据心绞痛发作时心电图一过性ST-T抬高的导联部位,判断痉挛靶血管的部位,论证这种判断的正确性,为介入处理(包括诊断与治疗)提供较为可靠的靶血管部位。结果 (1)男137例,女23例,男女比为5.96:1;男性与女性平均年龄分别为50.8±10.6岁与52.8±11.4岁(p>0.05);(2)160例中,痉挛靶血管部位为左主干4例(占2.5%)、左前降支88例(占55%)、左回旋支2例(占1.25%)、右冠状动脉46例(占28.75%)和多支冠状动脉20例(占12.5%)。结论本组160例VAP中,绝大多数为中年男性。半数以上的患者痉挛靶血管的部位在左前降支,其次为右冠状动脉,再次为多支冠脉(痉挛),左主干与左回旋支较少。VAP发作时,心电图一过性ST-T抬高是判断痉挛靶血管部位的一种简单易行的可靠方法。更多还原

【Abstract】 Objective To explore the clinical value of Electrocardiogram(ECG) in determining spasmodic targeted coronary artery in patients with variant angina pectoris(VAP).Methods One hundred and sixty patients diagnosed with VAP were collected.Spasmodic targeted coronary artery will be judged according to leads of transient ST-T elevation on ECG while angina pectoris onset.Through demonstration of the judgment,dependable targeted artery can be provided for Interventional therapy(including diagnosis and treatment).Results(1)One hundred and thirty seve n were male and 23 patients were female,the ratio was 5.91:1.The average age were 50.8±10.6(male),52.8±11.4(female)(p>0.05),respectively.(2)Spasmodic targeted coronary arteries were 4 cases(2.5%) on left main trunk,88 cases(55%) on left anterior descending artery,2 cases(1.25%) on left circumflex artery,46 cases(28.75%) on right coronary artery and 20 cases(12.5%) on multi-coronary arteries in all 160 VAP cases respectively.Conclusions(1)The majority of patients in this study were middle-aged male.(2)More than half of the spasmodic targeted coronary arteries were on left anterior descending artery.The second and third culprit arteries were on right and multi-coronary arteries.Left main and left circumflex arteries were rare.When angina pectoris occurs,transient ST-T elevation on ECG is an easy and dependable method to judge spasmodic targeted coronary artery.更多还原
