孔源性视网膜脱离外路术后黄斑下积液的OCT观察及其相关因素分析

【摘要】 目的对孔源性视网膜脱离(RRD)外路复位术后进行黄斑部相干光断层扫描(OCT)检查,观察术后黄斑区积液情况,并分析其相关因素。方法选择2015年8月至2016年12月到我院就诊,均采取巩膜扣带手术治疗且术后视网膜均复位成功的孔源性视网膜脱离患者,共有40例(40只眼),分别于术后半个月、1、3、6个月行最佳矫正视力(BCVA)、OCT检查。按积液情况及积液吸收时间长短分组,术后半个月黄斑下无积液,即无积液组;术后1个月黄斑下积液吸收,即1个月组;术后3个月黄斑下积液吸收,即3个月组;术后6个月黄斑下积液吸收,即6个月组,对其进行统计学分析。结果术后半个月黄斑下无积液19只眼(无积液组)。有积液21只眼,其中1个月组10只眼,3个月组6只眼,6个月组5只眼。手术前患者平均log MAR视力为1.29±0.72,术后各组平均logMAR视力分别为0.31±0.23、0.35±0.26、0.55±0.39、0.71±0.34,经单因素方差分析,各组视力较术前均有所提高,差异有统计学意义(P<0.05)。收集术后各组OCT扫描资料,经统计学分析得出,视网膜脱离范围和脱离时间为影响黄斑下积液吸收的因素(rs=0.544,rs=0.716)。结论视网膜脱离范围和脱离时间对术后黄斑下积液吸收有影响;黄斑下积液吸收时间长短可影响患者术后视力恢复,积液持续时间越长对患者视力损害越大。

【Abstract】 Objective To observe macular effusion in rhegmatogenous retinal detachment patients after scleral buckling surgery by using OCT,and to analyze the related factors. Methods Patients with rhegmatogenous retinal detachment who came to our hospital for treatment between August 2015 and December 2016,a total of 40 cases( 40 eyes),were enrolled. Patients were treated with sclera buckling surgery and the postoperative retina reattachment was successful. Patients were re-evaluated for best corrected visual acuity( BCVA) and were assessed with optical coherence tomography( OCT) at 15 days,1,3 and 6 months. ? According to the rate of fluid absorption,patients were divided into 4 groups-no effusion at 15 days,cleared at 1 month,3 months,and 6 months. Data were analyzed to compare between these groups.Results There were 19 eyes that had no effusion in macular area at 15 days. Effusion cleared later in other eyes,at 1month( 10 eyes),3 months( 6 eyes) and up to 6 months( 5 eyes). Mean log MAR was 1. 29 ± 0. 72 before the surgery and was 0. 31 ± 0. 23,0. 35 ± 0. 26,0. 55 ± 0. 39,and 0. 71 ± 0. 34 in the 4 groups,respectively. According to Single factor analysis of variance,BCVA improved in all groups( P < 0. 05). Analysis of OCT scanning data showed that the scope of retinal detachment and retinal detachment duration were associated with the absorption of the effusion in the macular area( rs = 0. 544,rs = 0. 716). Conclusions The scope of retinal detachment and retinal detachment duration are the main factors related to the absorption of the effusion in the macular area and the recovery of visual acuity.
