脑血氧饱和度与术后认知关系的Meta分析

【摘要】 目的采用系统评价方法,评价脑血氧饱和度对术后认知功能的影响。方法计算机从建库至2016年11月检索Pub Med、CNKI、Embase、维普、万方、Cochrane图书馆等数据库,全面收集符合纳入标准的随机对照试验(RCT)。2位研究人员,按照纳入与排除标准独立进行文献筛查,随后对入选文章进行评价。采用Rev Man 5.2进行meta分析。结果共纳入5项RCT研究,共计994例患者,确定符合纳入标准。术中脑血氧饱和度监测管理对POCD有影响,通过INVOS进行脑血氧饱和度监测和处理的干预组比不进行脑血氧饱和度监测的对照组术后7天出现COPD明显减低,差异具有统计学意义[Risk Ratio(M-H,Fixed,95%CI):0.43(0.33-0.56),P<0.00001]。结论术中监测并控制脑血氧饱和度能有效减少术后认知功能障碍的发生。

【Abstract】 Objective To evaluate the effect of cerebral oxygen saturation on postoperative cognitive function by systematic evaluation.Methods A randomized controlled trial(RCT) of Pub Med,CNKI,Embase,Weipu,Wanfan g,Cochrane Library were collected and collected from the database to November 2016.Two reviewers independently selected the literature according to the inclusion and exclusion criteria,extracted the data and evaluated the bias risk of inclusion,and used Rev Man 5.2 software for meta-analysis.Results Five RCT studies were included in a total of 994 patients,selected to meet inclusion criteria.Intraoperative cerebral oxygen saturation monitoring management has an impact on POCD,Intraoperative intervention by INVOS for cerebral oxygen saturation monitoring and treatment of the intervention group than did not carry out cerebral oxygen saturation monitoring of the control group 7 days after COPD significantly reduced,the difference has Statistical significance [Risk Ratio(MH,Fixed,95% CI): 0.43(0.33-0.56),P<0.000 01].Conclusion Intraoperative control of cerebral oxygen saturation can effectively reduce the occurrence of postoperative cognitive dysfunction.
