【摘要】 目的总结重症脑出血患者立体定向微创治疗的围手术期护理,通过周到细致的护理提高脑出血立体定向微创治疗后患者的生存率、致残率、生活质量。方法对48例患者进行术前护理和术后护理。在病情允许的情况下均在发病24小时~72小时内给予立体定向微创治疗,对患者头部进行CT定位扫描[1],并通过Leksell-G型立体定向系统计算出血肿中心的X、Y、Z坐标数值,确定穿刺点。在局麻下颅骨钻孔,将硬脑膜用单级电凝烧开一小孔,按照定位方向进针,将针芯拔出并使用硅胶管将少量血肿吸出,确认引流管在血肿腔当中后在切口旁将引流管穿出并固定,引流2-4天后拔管。结果术后3天复查CT结果显示血肿排出量90%以上38例,60%~90%之间10例。第一周内意识好转为43例(89.6%),无1例再出血。结论通过周密细致的护理,大大提高了脑出血患者的生存率;降低了致残率;提高了患者的生活质量,使患者用最短的时间回归家庭及社会更多还原

【Abstract】 Objective To summarize in severe cerebral hemorrhage patients with stereotactic minimally invasive treatment of perioperative nursing,through thoughtful and meticulous care to improve the survival rates of patients with cerebral hemorrhage after stereotactic minimally invasive treatment,morbidity,quality of life.Methods 48 patients with preoperative nursing and postoperative nursing.In the case of illness allows all 24-72 hours after onset of adept stereotactic minimally invasive hematoma puncture drainage,the Leksell-G stereotactic system,installation frame head CT scanning after positioning [1],through hematoma center for target to calculate the X,Y,Z coordinate values,the puncture point is determined.Skull drilling under local anesthesia,dura mater with singlestage electric coagulation boiling a small hole,the positioning direction into the needle,pull out after needle core by silicone tube pump out a small amount of hematoma,confirmed that the drainage tube in the hematoma cavity,will wear a fixed by incision drainage tube,drainage tube drawing in 2-4 days. Results The CT results showed that three days of postoperative hematoma discharge more than 90% of the 38 cases,10 cases were between 60% ~ 90%.Better awareness in the first week of 43 cases(89.6%),1 case was not bleeding. Conclusion Through the thorough careful nursing,greatly improved the survival rates of patients with cerebral hemorrhage; Reduce the morbidity.To improve the quality of life of patients,patients with the shortest possible time to return to family and society更多还原
