【摘要】 目的探讨TNM分期中c N0期甲状腺乳头状癌(PTC),两个年龄组别病人的患侧中央区淋巴结转移的差异。方法 cN0期PTC病人56例,根据TNM分期分成年龄<45岁组(29例)和年龄≥45岁组(27例),应用"卡纳琳"标识中央区淋巴结后行患侧预防性中央区淋巴结清扫(pCND),比较两组病人患侧淋巴结阳性率。结果年龄≥45岁组病人患侧清扫淋巴结(6.19±4.28)枚,阳性淋巴结(1.26±1.91)枚,阳性率20.4%;年龄<45岁组病人患侧清扫淋巴结(4.52±3.55)枚,患侧阳性淋巴结(1.14±1.60)枚,阳性率25.2%。两组患侧清扫淋巴结数、患侧阳性淋巴结数和阳性率比较,差异无统计学意义(P>0.05)。结论 cN0期PTC病人TNM分期的两个年龄组别中,患侧淋巴结阳性率无明显差异,病人是否进行患侧中央区淋巴结清扫,应形成一套综合评估体系。更多还原

【Abstract】 Objective To explore the metastases of the central lymph nodes in c N0 papillary thyroid carcinoma(PTC) through comparing two groups based on TNM staging.Methods A total of 56 cases with c N0 papillary PCT were enrolled,and divided into two groups(29 cases aged < 45 years old and 27 cases aged ≥45 years old) based on age in TNM staging." Ka Na Lin" was applied to identify the central lymph node after prophylactic central node dissection(p CND) on 56 patients,and the rates of positive lymph node were compared between the two groups.Results The number of lateral lymph node in the group aged ≥45 years old was 6.19 ± 4.28,of which,the number of positive lymph node was 1.26 ±1.91,thus the rate of positive lymph node was about 20.4%; the number of lateral lymph node in the group aged < 45 years old was 4.52 ± 3.55,of which,the number of positive lymph node was 1.14 ±1.60,thus the rate of positive lymph node was about 25.2%.There was no statistically significant difference between the two groups in the number of lateral lymph nodes,positive lymph nodes and their positive rates(P > 0.05).Conclusion The two groups with c N0 PTC in TNM stage had no difference in the positive rate of lateral lymph nodes.And a comprehensive assessment system should be developed on the lateral central lymph node dissection in the c N0 PCT.更多还原
