【摘要】 目的 探讨子宫内膜异位症不孕患者腹腔镜术后妊娠的影响因素。方法 对行腹腔镜手术的160例子宫内膜异位症合并不孕患者的临床资料进行回顾性分析,采用logistic回归分析法分析妊娠的相关影响因素。结果 本组患者术后妊娠105例,妊娠率为65.6%,其中自然妊娠82例(78.1%),辅助生育妊娠23例(21.9%)。术后≤1 a妊娠76例(72.4%),>1 a妊娠29例(27.60%);使用促性腺激素释放激素133例,其中妊娠97例,妊娠率72.9%。本组患者术后妊娠结局在经期天数、年龄、妊娠史、术后使用促性腺激素释放激素治疗、既往妇科炎症、宫腔操作史方面比较差异有统计学意义(P<0.05),疾病分期比较差异无统计学意义(P>0.05)。多因素回归分析显示,年龄、既往妊娠史、慢性妇科炎症、宫腔操作史、经期天数、术后使用促性腺激素释放激素治疗是子宫内膜异位症不孕患者术后妊娠结局的影响因素。结论 宫内膜异位症不孕患者腹腔镜术后妊娠结局受众多因素影响,应根据患者情况设计个体化治疗方案提高妊娠率。更多还原

【Abstract】 Objective To investigate the influence factors of gestation after laparoscopy in infertile women with endometriosis.Methods Clinic data of 160 cases of infertile women with endometriosis was retrospectively analyzed,related influence factors of gestation was analyzed by Logistic regression method.Results In this study,there were 105 cases of pregnancy after surgery,rate of pregnancy was 65.6%,in which 82 cases(78.1%) of natural pregnancy,23 cases(21.9%) of assisted reproduction.After surgery≤1 a,there were 76 cases(72.4%) of pregnancy,>1 a there were 29 cases(27.6%) of pregnancy; from 133 cases given gonadotropin for releasing hormone,there were 97 cases of pregnancy,rate of pregnancy was 72.9%.Difference of menstrual period days,age,pregnancy history,treatment of gonadotropin,gynecologic inflammation history,intrauterine operation history of pregnancy outcome between the two groups after surgery had statistic significance(P<0.05),difference of disease stage of the two groups had no statistic significance(P>0.05).Multi-factor regression analysis showed that,age,pregnancy history,gynecologic inflammation,intrauterine operation history,menstrual period days,treatment of gonadotropin for releasing hormone were influence factors of pregnancy outcome in infertile women with endometriosis.Conclusion Pregnancy outcome after laparoscopy in infertile women with endometriosis is affected by many factors,individualized therapy plans should be given to patients for increasing pregnancy rate.更多还原
