智能导航辅助CT引导下肺小结节穿刺活检的临床应用

【摘要】 目的比较穿刺导航机器人(ROBIO)辅助CT引导下肺小结节穿刺活检与传统CT引导下穿刺活检术的阳性率及并发症。方法 2014年1月至2016年12月99例诊断为肺小结节(直径≤3 cm)患者行穿刺活检术,随机分为ROBIO组和常规组。术后获得相应组织标本及病理诊断结果。术后随访时间3~35个月。所有资料进行统计学处理。结果病理诊断阳性率比较,ROBIO组93.5%,常规组75.5%,差异有统计学意义。ROBIO组7例患者出现了并发症,即不同程度的气胸;常规组15例患者出现并发症,差异无统计学意义。一次穿刺成功率分别为ROBIO组76.1%;常规组18.9%,组间有显著性差异。结论智能定位辅助系统能提高CT引导下肺部小结节穿刺活检阳性率,同时能减少穿刺次数,降低并发症发生率。

【Abstract】 Objective To compare the positive rate and complications in both biopsy locaziation robot( ROBIO) assisted biopsy guided by CT and conventional CT guided biopsy in lung small nodule. Methods 99 patients diagnosed as lung small nodule( ≤3 cm) from January 2014 to December 2016,are randomly distributed into two groups,ROBIO group and Conventional method group. All tissue samples and pathological diagnoses were collected after the procedure. Followup period was 3-35 months. All data were analysed statistically. Results Positive rate of pathological diagnoses in ROBIO group and Conventional method group was 93. 5% and 75. 5%,respectively,with significant differences. There were 7patients,who suffered from severe complications,i. e. pneumothorax,in ROBIO group and 15 in Conventional method group,without significance. One-shot biopsy was 76. 1% in ROBIO group and 18. 9% in Conventional method group,with significant difference. Conclusion Intelligent localization assisted system could improve positive rate for biopsy in lung small nodule,reduce the puncture shots,and lower complication rate.
