TurboHawk斑块旋切系统在下肢动脉硬化闭塞症的应用

【摘要】 目的:探讨TurboHawk斑块旋切术的临床应用价值。方法:收集2016-05~2017-03在我院血管外科应用TurboHawk斑块旋切系统治疗的8例动脉粥样硬化导致的下肢缺血施行TurboHawk斑块旋切,球囊扩张联合斑块旋切1例,斑块旋切联合支架植入2例,只使用斑块旋切系统切除者5例。进行患侧股动脉穿刺者6例,从对侧股动脉翻山者2例。术中均放置保护伞拦截可能脱落的斑块。结果:此8例患者至出院时均未发生再次堵塞,静息痛3例全部缓解,5例间歇性跛行均有改善。术后5例患者ABI值0.7~0.8,3例0.81~0.9,通过下肢动脉彩超及ABI随访这8例患者第1个月、第3个月,第6个月,其中有1例于术后20天再次狭窄入我院治疗,其余7例均无复发。结论:TurboHawk斑块切除技术是一种实用的介入治疗方法,其近期效果良好并值得推广。

【Abstract】 Objective: To explore the clinical application value of TurboHawk laque rotary resection. Methods:8 patients with atherosclerotic lower extremity ischemia received TurboHawk plaque excision were retrospectively analyzed between May 2016 and March 2017. One patient was treated with balloon expand and endovascular plaque excision. 2 cases were treated with stentand endovascular plaque excision,simple plaque excision in the lower extremity was applied to 5 patients. For the 8 patients,ipsilateral antegrade femoral artery puncture was carried out in 6 patients,contralateral retrograde femoral artery puncture was done in 2 patients. Protective umbrellas were placed during operation to intercept possible plaques. Results: No blockage occurred in all 8 patients until discharge. The rest pain was relieved in 3 cases,and intermittent claudication in 5 cases was improved. ABI value of 5 patients after operation was from 0. 7 to 0. 8,0. 81 ~ 0. 9 for 3 cases. The patients were followed up through color doppler ultrasound,ABI and walking distance between one month and nine months,mean( 5. 0 ± 0. 4) months. 1 case was readmitted into our hospital 20 days after operation,and no recurrence occurred in the other 7 cases. Conclusions:TurboHawk plaque excision technique is safe and effective and worthy of promotion in the future.
