血培养病原菌分布及耐药性分析

【摘要】 目的对新都区人民医院2015年1月至2016年1月血培养病原菌种类及耐药性进行分析,为指导临床抗菌药物合理使用提供依据。方法采用LABSTAR plus全自动血液培养仪对血培养瓶连续监测,对阳性标本及时涂片和转种,用Micro Scan Walk/Away-40plus全自动微生物鉴定和药敏试验分析系统对细菌进行鉴定和耐药性分析。结果排除8例假阳性干扰,1 570份血培养标本中分离出病原菌共135株,其阳性率为8.6%。其中有革兰阳性菌58株(42.96%),革兰阴性菌72株(53.33%),真菌3株(2.22%)。检出率高的病原菌包括大肠埃希菌、凝固酶阴性葡萄球菌。大肠埃希菌和肺炎克雷伯菌对美罗培南、头孢西丁、阿米卡星等敏感。凝固酶阴性葡萄球菌对万古霉素、莫西沙星敏感。结论该院的血培养病原菌种类复杂,耐药性差异较大,监测病原菌的种类及耐药性,有助于临床合理用药,避免抗菌药物滥用。

【Abstract】 Objective To analyze the distribution and drug resistance of pathogenic bacteria in blood culture from January 2015 to January 2016 to provide the basis for guiding clinical rational use of antimicrobial agents.Methods The LABSTAR plus fully automatic blood culture instrument was adopted to conduct the continuous monitoring on blood culture bottles.The positive specimens were timely smeared and switched to cultivation.The bacteria identification and drug resistance analysis were performed by using the Micro Scan Walk/Away-40 plus automatic microbial identification and drug susceptibility test analysis system.Results Among 1 570 specimens of blood culture,8cases of false positive interference were excluded and 135 strains of pathogenic bacteria were isolated,the positive rate was 8.6%,including 58strains(42.96%)of Gram-positive bacteria,72strains(53.33%)of Gramnegative bacteria and 3strains(2.22%)of fungi.Pathogenic bacteria with high detection rate included Escherichia coli,coagulasenegative Staphylococci.Escherichia coli and Klebsiella pneumoniae were sensitive to meropenem,cefoxitin,amikacin,and so on.Coagulase-negative Staphylococci were sensitive to vancomycin and moxifloxacin.Conclusion The kinds of pathogenic bacteria in blood culture of this hospital are complex and drug resistance difference is larger.Monitoring the types and drug resistance of pathogenic bacteria contributes to rational drug use in clinic and avoid the abuse of antibiotic drugs.
