交锁髓内钉联合锁定钢板固定与单纯锁定钢板固定治疗股骨骨折骨不连的临床疗效对比

【摘要】 目的 :探讨交锁髓内钉联合锁定钢板固定与单纯锁定钢板固定治疗股骨骨折后骨不连的临床疗效。方法 :选取2011年1月~2014年12月在我院治疗的股骨骨折后骨不连患者80例,随机分为交锁髓内钉联合锁定钢板固定组与单纯锁定钢板固定组各40例,并且分别对两组患者随访12个月。观察两组患者手术时间,术中出血量,住院天数、骨折愈合时间、膝关节的功能评分,术后并发症发生率。结果 :交锁髓内钉联合锁定钢板固定与单纯锁定钢板固定组的平均手术时间分别为108.39±14.7,63.68±15.9min,术中平均出血量分别为413.25±19.8,240.78±18.2m L;住院时间分别为15.27±2.2,10.48±1.8天;骨折愈合时间为16.5±4.1,21.8±5.8天,术后并发症发生率分别为22.5%(12/40),5%(3/40)。结论 :本次研究表明,单纯锁定钢板固定治疗股骨骨折的手术时间较短、术中失血量较少,住院时间短。但是交锁髓内钉联合锁定钢板在治疗中具有术后骨折愈合较快,总并发症发生少,膝关节功能恢复良好等优点。

【Abstract】 Objective To observation the clinical curative effect of combine interlocking intramedullary nail and dynamic compression plate fixation versus dynamic compression plate fixation for nonunion after femoral fracture. Methods In this study, we retrospective review 80 patients with nonunion after femoral fracture. Forty patients were treated with combine interlocking intramedullary nail and dynamic compression plate fixation, the others were treated with dynamic compression plate fixation. To value relevant index of the two groups during eighteen moths’ follow up. Results The operative time of combine interlocking intramedullary nail and dynamic compression plate fixation versus dynamic compression plate fixation was respectively108.39±14.7, 63.68±15.9minmin; blood loss was 413.25±19.8, 240.78±18.2ml; the hospital stay was respectively 15.27±2.2, 10.48±1.8 days; fracture healing time was respectively 16.5±4.1, 21.8±5.8; the complication rate was22.5%(12/40), 5%(3/40). Conclusion These findings suggest that compared with the combine interlocking intramedullary nail and dynamic compression plate fixation, dynamic compression plate in treatment of femoral bone nonunion fracture can obtain shorter operative time, lesser blood loss, shorter hospital stay. But the combine interlocking intramedullary nail and dynamic compression plate fixation achieved an shorter fracture healing time, less complication rate, better knee function.
