术中保温对老龄大鼠术后认知功能影响的实验研究

【摘要】 目的探讨术中保温护理对老龄大鼠术后认知功能障碍的影响。方法将30只18月龄雄性SD大鼠随机分为对照组、未保温组和保温组各10只。对照组未予以手术干预;未保温组手术过程中置于室温环境,未作保温处理;保温组手术过程中予以保温毯保温。结果与对照组比较,未保温组术中体温及血压显著降低,术后第7天Toll样受体4、髓样分化因子88和核转录因子κB P65表达水平显著提高;且术后第14天NeuN免疫荧光染色检测海马CA1区神经元数量减少,Morris水迷宫实验平台跨越次数、目标象限路径百分比、目标象限时间百分比显著降低(均P<0.05)。与未保温组比较,保温组术后第7天Toll样受体4、髓样分化因子88和核转录因子κB P65表达上调程度降低,术后第14天海马CA1区神经元丢失数量减少,Morris水迷宫实验学习和记忆指标改善(均P<0.05)。结论术中保温通过减轻老龄大鼠脑内免疫应激反应,改善术后认知功能障碍。

【Abstract】 Objective To explore the effect of intraoperative warming on postoperative cognitive dysfunction in aged rats.Methods A total of 30eighteen-month-old male SD rats were randomized into a control group,a No-Keeping Warm(No-KW)group and a Keeping Warm(KW)group,with 10 rates in each group.The control group did not receive operation;the No-KW group was under the room temperatue envrionment in the operation,without keeping warm;the KW group was warmed with thermal insulation blanket in the operation.Results When the No-KW group compared with the control group,the intraoperative temperature and blood pressure were significantly lower,while the expression of Toll-like Receptor 4(TLR4),Myeloid differentiation factor 88(MyD88)and transcription factors-κB(NF-κB)P65in the 7th day after the operation were significantly higher;and in the 14 th day,the number of neuron in hippocampus CA1 region measured by NeuN immunofluorescence,and the platform crossing times,the percent distance and the percent time in the target quadrant via Morris Water Maze test were significantly less(P<0.05 for all).When the KW group compared with the No-KW group,the increase of the expression of TLR4,MyD88 and NF-κB P65 in the 7th day were significantly less,and in the 14 th day,the lost of neuron in hippocampus CA1 region was less,and the learning and memory function were significantly improved(P<0.05 for all).ConlusionIntraoperative warming could improve postoperative cognitive dysfunction in aged rats by reducing immunization stress response.
