【摘要】 目的:探索三级甲等综合性医院手术室节假日急诊及批量伤的科学排班模式方法。方法:2015年度和2016年度节假日在我院手术室实施三阶梯排班模式,即值班第一梯队、备班第二梯队、突发事件应急排班第三梯队,以及建立各班次的职责和关联科室的无缝连接制度和流程。结果:2015年度和2016年度节假日共进行急诊手术13 295台。近两年度我院节假日放假共58天,共安排护士505人次。全年节假日科室无护理差错事故发生,手术安全运行率为100%。实施后手术室护士对科室节假日值班人员能级结构、加班人员到岗及时性、排班方法的科学性、实施以后收效等方面评价均优于实施前差异有统计学意义(P<0.05)。结论:三阶梯排班模式能按层次科学合理安排人力资源,既能确保手术值班现场的安全,又能确保处置急诊和批量伤员的及时性,保证手术室工作有序科学运行,值得同行借鉴。更多还原

【Abstract】 Objective: To explore the scientific shift model method for emergency and batch injury in operating room of the third-grade class-A comprehensive hospital during holidays. Methods: In the holidays of 2015 and 2016,the three-step shift model was implemented in our operating room during holidays,namely the first team on duty,the second team on duty and the third team for emergency. The duties of all teams as well as seamless connecting system and procedures for related departments were established. Results: A total of 13 295 emergency operations were conducted in holidays of 2015 and2016. In the past two years,our hospital holidays were only 58 days and within this time we arranged a total of 505 nurses on duty. There were no care errors in the department during holidays and weekends. The operation safety rate was 100%. After the implementation,the evaluations of nurses in the operating room about staff duty level structure,timeliness of overtime personnel,scientific level of scheduling mode and the were better than that before the implementation. The difference was of statistical significance( P < 0. 05). Conclusions: The three-step shift model can arrange the human resources according to the scientific and reasonable level,which can ensure the safety of the operation on duty and ensure the timeliness of the emergency and batch casualties,thus ensuring the scientific operation of the operating room.
