呼吸兴奋剂配合无创呼吸机对COPD合并Ⅱ型呼吸衰竭患者血气指标及炎症因子的影响

【摘要】 目的观察呼吸兴奋剂配合无创呼吸机对慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者血气指标及炎症因子的影响。方法选取收治的COPD合并Ⅱ型呼吸衰竭患者300例作为研究对象,按照随机数字表法分为研究组和对照组各150例,其中对照组给予呼吸兴奋剂治疗,研究组在对照组基础上配合无创呼吸机治疗。观察两组临床疗效,血气指标二氧化碳分压(PaCO2)、氧分压(PaO2)以及酸碱度(PH),炎症因子高敏C反应蛋白(hs-CRP)、白介素-18(IL-18)、前列腺素E2(PGE2)水平。结果研究组总有效率88.00%显著高于对照组68.67%(P<0.05);治疗后两组PaCO2显著低于治疗前(P<0.05),且研究组显著低于对照组(P<0.05),两组PaO2、PH水平显著高于治疗前(P<0.05),且研究组显著高于对照组(P<0.05);治疗后两组hs-CRP、IL-18、PGE2水平显著低于治疗前(P<0.05),且研究组显著低于对照组(P<0.05)。结论采用呼吸兴奋剂配合无创呼吸机能够显著改善COPD合并Ⅱ型呼吸衰竭患者血气指标及炎症因子水平,提高治疗效果。

【Abstract】 Objective To explore the influence of respiratory stimulant combined with noninvasive ventilator on blood gas index and inflammatory factors of patients with Chronic obstructive pulmonary disease(COPD) complicated with type Ⅱ respiratory failure.Methods 300 cases of patients with COPD complicated with type Ⅱ respiratory failure treated in our hospital were selected as the study objects,and were divided into study group and control group according to the random number table method,150 cases in each group.The control group were treated with respiratory stimulant,and the study group were combines with noninvasive ventilator on the basis of the control group.Clinical effect,partial pressure of carbon dioxide(PaCO2),blood oxygen partial pressure(PaO2) and p H(PH),inflammatory factors of high sensitive C reactive protein(hs-CRP) and interleukin-18(IL-18) and prostaglandin E2(PGE2) levels were observed in the two groups.Results The total effective rate of the study group was 88.00%,significantly higher than that of the control group(P<0.05).After treatment,PaCO2 were significantly lower than that before treatment in the two groups(P<0.05),which of the study group were significantly lower than that of the control group(P<0.05).PaO2 and PH levels were significantly higher than those before treatment(P<0.05),which of the study group were significantly higher than those of the control group(P<0.05).After treatment,the levels of hs-CRP,IL-18 and PGE2 were significantly lower than those before treatment in the two groups(P<0.05),which in the study group were significantly lower than those in the control group(P<0.05).Conclusions Respiratory stimulant combined with noninvasive ventilator can significantly improve blood gas index and inflammatory factors of patients with COPD complicated with type Ⅱ respiratory failure,and can improve the treatment effect.
