【摘要】 目的研究甲胎蛋白(AFP)、甲胎蛋白异质体(AFP-L3)在原发性肝癌患者血清中的表达及意义。方法分别采用电化学发光法、酶联免疫吸附实验(ELISA)测定80例原发性肝癌(原发性肝癌组)、80例良性肝病(良性肝病组)及80例健康体检者(健康对照组)血清AFP、AFP-L3含量及检出阳性率,分析AFP-L3与肿瘤大小、临床分期、组织分化程度的关系。结果与健康对照组比较,良性肝病组和原发性肝癌组血清AFP、AFP-L3含量、检出阳性率显著升高,差异有统计学意义(P<0.05);良性肝病组和原发性肝癌组比较,差异有统计学意义(P<0.05)。AFPL3检出阳性率随肿瘤直径增加、临床分期进展逐渐升高,差异有统计学意义(P<0.05),而不同组织分化程度间比较,差异无统计学意义(P>0.05)。原发性肝癌组腹腔积液脱落癌细胞、肝组织AFP、AFP-L3含量和检出阳性率比较,差异均无统计学意义(P>0.05)。联合检测血清AFP、AFP-L3诊断原发性肝癌的敏感性为86.25%,特异性为93.75%。结论 AFP-L3在原发性肝癌的早期诊断中具有一定临床价值。更多还原

【Abstract】 Objective To investigate the expression and clinical significance of serum alphafetoprotein( AFP) and alphafetoprotein variant( AFP-L3) in patients with primary liver cancer. Methods The serum levels of AFP and AFP-L3 as well as positive detection rate were detected by electrochemiluminescence method and enzyme-linked immunosorbent assay( ELISA),respectively in 80 cases of primary hepatocellular carcinoma( primary liver cancer group),80 cases of benign liver diseases( benign liver disease group) and 80 healthy subjects( control group). Moreover the correlation between serum AFP-L3 and tumor size,cinical staging,tissue differentiation degree was analyzed. Results As compared with those in control group,the serum levels of AFP and AFP-L3 as well as positive detection rate in benign liver disease group and primary liver cancer group were significantly increased( P < 0. 05),moreover,there were significant differences between benign liver disease group and primary liver cancer group( P < 0. 05). The positive detection rates of AFP-L3 were closely correlated to tumor size and clinical stages( P < 0. 05),however,there were no significant differences between different tissue differentiation degree( P > 0. 05). Besides there were no significant differences in seroperitoneum abscission cancer cells,the levels of AFP,AFP-L3 in liver tissues and positive detection rates in primary liver cancer group( P > 0. 05). The sensitivity and specificity AFP by combination detection of serum levels of AFP and AFP-L3 in diagnosis of primary liver cancer were 86. 25% and 93. 75%,respectively. Conclusion The detection of AFP-L3 has certain clinical value in early diagnosis of primary hepatocellular carcinoma.更多还原
