【摘要】 目的观察颅脑外伤后头痛患者脑血流和血流动力学的变化规律。方法选取石家庄市中医院于2013年1月—2014年12月的颅脑外伤恢复期头痛患者40例为观察组,同时选取40例健康体检者作为对照组,观察两组的大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)、基底动脉(BA)和椎动脉(VA)的血流速度变化以及全血黏度、血浆黏度、红细胞压积、红细胞变形指数和纤维蛋白原含量。结果观察组全血黏度、血浆黏度、红细胞压积和纤维蛋白原含量均高于对照组,红细胞变形指数低于对照组(P<0.05,P<0.01)。观察组MCA、ACA、PCA、VA和BA的收缩期峰流速、舒张末期血流速度及平均血流速度均较对照组减慢(P<0.05)。结论颅脑外伤后恢复期头痛患者存在脑血流和血流动力学变化,表现为血液黏度增高、血液瘀滞、脑血流速度减慢,可用于指导临床。更多还原

【Abstract】 Objective To observe changes of cerebral hemodynamics and hemorheology of patients with headache after craniocerebral trauma. Methods A total of 40 patients with headache during convalescent period of craniocerebral trauma admitted during January 2013 and December 2014 were selected as observation group,while other 40 healthy people taking medical examination at the same period were selected as control group. Changes of blood flow velocity of middle cerebral artery( MCA),anterior cerebral artery( ACA),posterior cerebral artery( PCA),basilar artery( BA) and vertebral artery( VA),whole blood viscosity,plasma viscosity,hematocrit,erythrocyte deformation index and fibrinogen content were observed in two groups. Results In observation group,values of whole blood viscosity,plasma viscosity,hematocrit and fibrinogen content were significantly higher,while value of erythrocyte deformation index was significantly lower than those in control group( P < 0. 05,P < 0. 01). The systolic peak velocity,end diastolic blood flow velocity and mean blood flow velocity of MCA,ACA,PCA,VA and BA in observation group were significantly slower than those in control group( P < 0. 05). Conclusion Changes of cerebral hemodynamics and hemorheology of patients with headache after craniocerebral trauma are enhanced blood viscosity,blood stasis and slow cerebral speed of blood flow,and it can be used to guide clinical treatment.更多还原
