EMP2、TTF-1在子宫内膜增生症和子宫内膜样腺癌中的表达及意义

【摘要】 目的探讨上皮膜蛋白2(EMP2)与甲状腺转录因子1(TTF-1)在子宫内膜增生症和子宫内膜样腺癌中的表达及其临床意义。方法选取1990年1月至2014年10月间在中山市人民医院妇科就诊、起初病理诊断为子宫内膜增生症(包括单纯型、复杂型、不典型增生),经过10年随访发展和没有发展为子宫内膜癌的患者标本107例,其中正常子宫内膜标本28例,子宫内膜增生症癌变标本51例,子宫内膜癌标本28例,应用免疫组化S-P法检测各组标本中EMP2、TTF-1表达情况。结果在正常增生期子宫内膜、子宫内膜增生症、子宫内膜样腺癌组织中EMP2蛋白的阳性表达率分别为10.7%、33.3%、82.1%,依次递增,TTF-1的阳性表达率分别67.9%、45.1%、35.7%,依次递减,差异均有统计学意义(P<0.05);EMP2阳性者的比例随手术病理分期、组织病理分级的升高而增加,差异有统计学意义(P<0.05),而TTF-1阳性者的比例随手术病理分期、组织病理分级的升高而逐渐下降,差异有统计学意义(P<0.05)。结论 EMP2、TTF-1可能参与子宫内膜增生发展至子宫内膜癌的过程,对子宫内膜癌的早期诊断及预测预后有一定的价值。

【Abstract】 Objective To investigate the expression and clinical significance of epithelial membrane protein-2(EMP2) and thyroid transcription factor-1(TTF-1) in endometrial hyperplasia and endometrial adenocarcinoma.Methods A total of 107 cases of patients, who admitted to Department of Gynecology of Zhongshan People’s Hospital and initially confirmed as by endometrial hyperplasia(including simple hyperplasia, complex hyperplasia, atypical hyperplasia) by pathology diagnosis from January 1990 to October 2014, were selected as the research subjects. After ten years of clinical follow-up, among of 107 cases of specimens, there were 28 cases of normal endometrial specimens, 51 cases of endometrial hyperplasia specimens, 28 cases of endometrial adenocarcinoma. The expression of EMP2 and TTF-1were detected by immunohistochemical S-P method. Results The positive expression rate of EMP2 in normal proliferative endometrium, endometrial hyperplasia(simple hyperplasia, complex hyperplasia, atypical hyperplasia), and endometrial adenocarcinoma tissue were 10.7%, 33.3% and 82.1%, respectively, with an increasing trend; the positive expression rate of TTF-1 in three different tissues were 67.9%, 45.1% and 35.7%, respectively, with a decreasing trend(P<0.05). The proportion of EMP2 positive patients increased with increase of surgical pathology and histopathological grade(P<0.05), while the proportion of TTF-1 positive patients decreased with increase of surgical pathology and histopathological grade(P<0.05). Conclusion EMP2 and TTF-1 may be involved in the occurrence and development from endometrial hyperplasia to endometrial carcinoma, which may be valuable for the early diagnosis and prognosis of endometrial carcinoma.
