踝关节正侧位加足正斜位X线片在诊断腓骨远端骨骺损伤的价值
Diagnostic value of ankle anteroposterior and lateral views combined with foot anteroposterior and oblique views in distal fibular epiphyseal injuries
【摘要】 目的对比分析踝关节正侧位联合足正斜位X线片与单独踝正侧位X线片和足正斜位X线片诊断腓骨远端骺损伤的临床效果。方法选取2014年1月至2015年5月,于我院急诊就诊的踝关节扭伤并伴有外踝肿胀的患者196例,其中男100例,女96例;年龄3~15岁,平均(9.0±5.8)岁;病程2~18 h,平均(10±8.2)h。统计如果只摄踝关节正侧位X线片和只摄足正斜位X线片或者两种方法联合的敏感度、特异度、准确度。结果通过对比以上3种方法的诊断的各项指标发现,踝关节正侧位联合足正斜位X线片的准确率为92.3%,单独摄踝正侧位X线片的准确率为72.4%,单独摄足斜位X线片的准确率为86.7%。结论相较于只摄踝关节正侧位X线片和只摄足正斜位X线片,踝关节正侧位联合足正斜位与踝正侧位具有明显的诊断优势。
【Abstract】 Objective To compare the clinical effects of ankle anteroposterior and lateral views combined with foot anteroposterior and oblique views with only ankle anteroposterior and lateral views and only foot anteroposterior and oblique views in the diagnosis of distal fibular epiphyseal injuries, and to confirm that plaster external fixation is needed for the patients whose distal fibular epiphyseal injuries are clearly diagnosed. Methods A total of 196 patients who had ankle sprain or lateral malleolus swelling from January 2014 to May 2015 were selected. There were 100 males and 96 females, whose mean age was( 9.0 ± 5.8) years old( range: 3- 15 years old). The mean course of disease was( 10 ± 8.2) hours( range: 2- 18 hours). The sensitivity, specificity and accuracy were counted by only taking ankle anteroposterior and lateral views, only taking foot anteroposterior and oblique views or both 2 kinds of views. Results Through the comparison of the indicators for diagnosis in the above 3 methods, the correct rates of the ankle anteroposterior and lateral views combined with foot anteroposterior and oblique views, only the ankle anteroposterior and lateral views and only the foot anteroposterior and oblique views were 92.3%, 72.4% and 86.7%. Conclusions Compared with only the ankle anteroposterior and lateral views and only the foot anteroposterior and oblique views, the method of combined both 2 views is superior.
