鲍曼不动杆菌的临床分布及耐药性变迁

【摘要】 目的:了解本院鲍曼不动杆菌(Acinetobacter baumannii,AB)的临床分布特点以及耐药性变迁,为临床有效预防和治疗AB感染提供依据。方法:回顾性分析本院2014年1月~2015年12月检出的1173 AB的标本来源、科室分布以及耐药性变迁。结果:2014年和2015年均以AB检出最多,分别占14.70%、16.94%。AB标本来源主要是痰(1013株,86.36%)。科室分布主要来自重症监护病房(intensive care unit,ICU)(综合ICU和心胸脑外科ICU)(686株,58.48%),其次是呼吸内科(105株,8.95%)、心血管内科(66株,5.63%)。ICU的多重耐药鲍曼不动杆菌(Multidrug-resistant Acinetobacter baumannii,MDR-AB)的检出率显著高于非ICU,差异有统计学意义(P<0.01)。14种常用抗菌药物的药敏结果显示:ICU除头孢噻肟外,AB耐药率总体呈上升趋势,差异均有统计学意义(P<0.01);非ICU除头孢吡肟外,AB耐药率总体呈上升趋势,差异均有统计学意义(P<0.05)。结论:AB的临床分布以ICU病房最多,且耐药性最强。

【Abstract】 Objective: To investigate the clinical distribution and drug resistance of Acinetobacter baumannii in The First People’s Hospital of Guiyang,so as to provide the basis for the effective prevention and treatment of Acinetobacter infection in Bauman. Methods: A retrospective analysis was made on the source,distribution and drug resistance of 1173 Acinetobacter baumannii isolated from January 2014 to 2015 in The First People’s Hospital of Guiyang in December. Results: The maximum was detected by AB,accounting for 14. 70% and16. 94% respectively in 2014 and 2015. The main source of Acinetobacter baumannii( Bauman) was sputum( 1013 strains,86. 36%). The distribution of the Departments was mainly from ICU( Comprehensive ICU and cardiothoracic surgery ICU)( 686 strains,58. 48%),respiratory medicine( 105 strains,8. 95%) and cardio-vascular medicine( 66 strains,5. 63%). The detection rate of ICU multidrug-resistant Acinetobacter baumannii was significantly higher than that of non ICU,the difference was statistically significant( P < 0. 01). 14 sensitivity of antibacterial drugs showed that: In addition to cefotaxime,Bauman real bacteria resistance rate had overall upward trend,the differences were statistically significant( P< 0. 01) in ICU; In addition to cefepime,Bauman Acinetobacter resistance rate had overall upward trend,the differences were statistically significant( P<0. 05) in non ICU. Conclusion: The clinical distribution of AB is the largest in ICU ward with the most resistant to Bauman.
