【摘要】 目的研究跟骨锁定钢板治疗跟骨严重粉碎性骨折的临床效果。方法选取2014年9月~2016年6月我院收治的48例跟骨粉碎性骨折患者进行观察,按随机数字表法分成A、B两组,每组31例,A组采用锁定钢板内固定;B组采用非锁定钢板内固定。观察记录两组患者手术术中的出血量、以及皮缘坏死、切口感染、腓肠外侧皮神经损伤等的并发症发生率。并根据Maryland足部评分标准对所有患者进行评价。结果A组并发症的发生率为4.17%,低于B组16.67%,但两组并发生症发生率差异无统计学意义(P>0.05)。A组优14(58.33%)例,良6(25.00%)例,中3(12.50%)例,差1(4.17%)例,优良率83.33%。B组优9(37.50%)例,良4(16.67%)例,中7(29.17%)例,差4(16.67%)例,优良率54.17%。A组优良率明显高于B组,差异有统计学意义(P<0.05)。A组手术时间、术中出血量均明显少于B组,差异有统计学意义(P<0.05),但两组术后Bohler角及Gissane角比较,差异无统计学意义(P>0.05)。结论跟骨锁定钢板治疗跟骨严重粉碎性骨折疗效果良好,值得推广应用。更多还原

【Abstract】 Objective To study the clinical effect of calcaneal locking plate in the treatment of severe calcaneus comminuted fracture of calcaneus.Methods 48 cases of comminuted fracture of calcaneus in our hospital from September 2014 to June 2016 were selected and observed.They were randomly divided into A group and B group,31 cases in each group.Patients in A group were treated with locking plate internal fixation.Patients in B group were treated with non locking plate fixation.The amount of bleeding in patients with surgery were recorded in the two groups,as well as skin necrosis,wound infection,the incidence rate of the lateral sural cutaneous nerve injury complications.According to the Maryland foot standard for evaluation,all patients were evaluated.Results The incidence of complications in A group was 4.17%,which was lower than that in B group,but there was no significant difference between the two groups(P > 0.05).There were excellent in 14 cases(58.33%),good in 6 cases(25%),medium in 3 cases(12.50%),poor in 1 case(4.17%) in A group,the excellent good rate was 83.33%.There were excellent in 9 cases(37.50%),good in 4 cases(16.67%),midium in 7 cases(29.17%),poor in 4 cases(16.67%) in B group,the excellent good rate was 54.17%.A group was obviously higher than B group,the difference was statistically significant(P < 0.05).The operation time,intraoperative blood loss in group A were significantly less than those in group B,the differences were statistically significant(P < 0.05).But the Bohler angle and Gissane angle in both groups had no significant difference(P > 0.05).Conclusion Calcaneus Locking plate for the treatment of severe comminuted calcaneal fracture has good treatment effect.It is worthy of popularization and application.更多还原
