二甲双胍对2型糖尿病合并亚临床甲状腺功能减退患者血清促甲状腺激素水平的影响
Effect of metformin on serum thyroid stimulating hormone levels in patients with type 2 diabetes mellitus and subclinical hypothyroidism
【摘要】 目的:研究二甲双胍对2型糖尿病合并亚临床甲状腺功能减退患者血清促甲状腺激素(TSH)水平的影响。方法:选择2014年6月至2015年12月在西安市华山中心医院诊治的2型糖尿病合并亚临床甲状腺功能减退患者80例,随机分为观察组和对照组,每组40例。观察组采用二甲双胍治疗,对照组采用阿卡波糖治疗;比较治疗前、后两组的空腹血糖(FBG)值、餐后2 h血糖(2 h PBG)值、血清总甲状腺素(TT4)、总三碘甲腺原氨酸(TT3)、游离甲状腺素(FT4)、游离三碘甲腺原氨酸(FT3)和TSH的水平。结果:治疗后,两组的FBG和2 h PBG水平均明显降低(均P<0.05),且观察组降低更为明显(P<0.05);治疗前、后两组的血清TT4、TT3、FT4和FT3水平均无明显差异(均P>0.05),治疗后观察组的TSH水平明显降低(P<0.05)。结论:二甲双胍不仅能有效控制2型糖尿病合并亚临床甲状腺功能减退患者的血糖,还能抑制血清TSH的水平,可用于甲状腺疾病的治疗。
【Abstract】 Objective: To investigate the effect of metformin on serum thyroid stimulating hormone(TSH)level in patients with type 2 diabetes mellitus and subclinical hypothyroidism. Methods: 80 patients with type 2 diabetes mellitus and subclinical hypothyroidism treated in our hospital from June 2014 to December 2015 were selected and divided into two groups randomly. The patients in the observation group were treated with metformin,and those in the control group were treated with acarbose. The fasting blood-glucose(FBG),2 h postprandial blood-glucose(2 h PBG),serum total thyroxine(TT4),total triiodothyronine(TT3)and free thyroxine(FT4),free triiodothyronine(FT3)and TSH levels were measured and compared. Results: After treatment,the FBG and 2 h PBG were notably decreased(P<0. 05),and the difference was more significant in the observation group(P<0. 05). Before and after treatment,there were no significant differences in the serum levels of TT4,TT3,FT4 and FT3 between the two groups(P>0. 05),but the TSH level of the observation group was obviously lower than that of the control group after treatment(P<0. 05).Conclusion: Metformin could effectively control blood-glucose and inhibit the serum TSH level in patients with type 2 diabetes mellitus and subclinical hypothyroidism. It could be used for the treatment of thyroid disease.
