【摘要】 目的:探讨持续冲洗联合负压封闭引流(VSD)治疗胰瘘的临床效果。方法:选取2008年1月至2015年12月广西医科大学第六附属医院收治的胰瘘患者36例,其中以应用持续冲洗联合VSD治疗的患者19例作为治疗组,以双腔套管持续冲洗引流治疗的患者17例作为对照组,观察比较两组患者临床疗效、随访情况及切口感染发生情况。结果:治疗组在瘘口愈合时间、平均住院时间、换药次数、切口感染率方面均优于对照组(P<0.05)。两组病死率比较,差异无统计学意义(P>0.05)。两组患者出院后均随访1年以上,大部分患者恢复良好,无胰瘘相关并发症发生,对照组有1例胰腺假性囊肿形成,体积不大,无症状。结论:持续冲洗联合VSD治疗胰瘘的临床效果较好,患者恢复快,切口感染率低,有较好的临床应用价值。更多还原

【Abstract】 Objective:To investigate the clinical application of continuous irrigation combined with vacuum sealing drainage(VSD)for pancreatic fistula.Methods:36patients with pancreatic fistula admitted in our hospital from January 2008 to December 2015 were selected and divided into a treatment group(n =19)and a control group(n =17).The patients in the treatment group received continuous irrigation combined with VSD,while those in the control group received double-lumen tube irrigation and drainage.The clinical effect and the infection rate of incisional wound were compared between the two groups.Results:The healing time,average hospital days,dressing change frequency and incision infection rate in the treatment group were superior to those in the control group(P <0.05).There was no statistical significance in the mortality between the two groups(P >0.05).All patients were followed up for above one year after discharge.The majority of patients recovered well and no pancreatic fistula related complications was found.One case of pancreatic pseudocyst in the control group was found,but the cyst was small and asymptomatic.Conclusion:Continuous irrigation combined with VSD could achieve good therapeutic effect on pancreatic fistula patients,with less infection and speedy recovery.更多还原
