【摘要】 目的比较Morse跌倒评估量表、Hendrich II跌倒风险模型及老年人跌倒风险评估工具三种不同的量表在老年住院人群中跌倒风险评估的价值。方法对两家综合性医院的住院老年患者分别进行三种量表的跌倒风险评估。根据近一年来有无跌倒史,将患者分为跌倒组和无跌倒组,采用独立样本的t检验分析各个量表在两组间的分值差异,并计算各量表的敏感度、特异度、误诊率、漏诊率、Youden指数、阳性预测值、阴性预测值及ROC曲线,从而比较三个量表在研究对象中的应用价值。结果共纳入研究对象386例,其中跌倒组为64例,无跌倒组为322例。MFS、HFRM和FRASE三个量表在两组间的得分差异均有统计学意义(P<0.05)。三种量表的ROC曲线下面积分别为:0.835、0.853和0.725。结论三个量表均可应用于老年住院人群的跌倒风险筛查,在此研究人群中HFRM具有最好的预测价值,其次为MFS,较差的为FRASE。更多还原

【Abstract】 Objective To compare the application values of three different fall risk assessment measures including Morse Fall Scale(MFS), Hendrich-II Fall Risk Model(HFRM) and the Fall Risk Assessment Scale for the elderly(FRASE) in elderly inpatients. Methods These three fall risk assessment measures listed above were tested among elderly inpatients in two general hospitals. We categorized all patients into the fall group and the non-fall group based on their falling status(yes or no) in the past year. T-test of the independent samples was used to compare scores of different measures between two groups. We further calculated sensitivity, specificity, misdiagnosis rate, missed diagnosis rate, Youden index, positive predictive value, negative predictive value and ROC curve for each scale measure, to compare the application values of three measures. Results A total of 386 cases were included in the study(64 in the fall group and 322 in the non-fall group). The scores obtained from the two groups were significantly different(P<0.05) in both measures(MFS, HFRM, and FRASE). The area under the ROC curve was 0.835, 0.853, and 0.725 respectively. Conclusions Three different measures can both be applied for the fall risk screening of elderly inpatients, with HFRM the best perform and FRASE the poorest.更多还原
