多孔钽棒植入股骨头塌陷风险与生物力学支撑效果分析

【摘要】 目的对研究钽棒植入手术的适宜力学参数进行细化分析,为临床应用提供科学依据。方法利用CT扫描数据建立了包含钽棒-骨质接触面的有限元分析模型,其中钽棒和股骨头坏死区采用理想弹塑性材料模拟,采用通用有限元分析软件ANSYS对多孔钽棒植入后的股骨头塌陷量进行求解。结果当坏死区域角≥120°时股骨头产生显著局部塌陷,且钽棒植入体滑移微动量增大使钽棒支撑失效。结论股骨头坏死区域过大时不宜采用钽棒植入术。

【Abstract】 Objective To analyze and determine proper parameters of tantalum rod implantation for femoral head necrosis for clinical application. Methods Finite element model including tantalum rod to bone substance contact surfaces was built and the tantalum rod and bone substance of necrosis area were simplified as ideal elastic-plastic materials. Results Collapse of femoral head was solved considering different central angle β of necrosis area in the ANSYS software. The analysis result indicated that significant femoral head local collapse generated when β was greater or equal to 120 degree and micromotions between tantalum rod and bone substance of necrosis area was also increasing, that raised the risk of bracing failure.Conclusion Implantation of tantalum rod is not suggested when necrosis area of femoral head is too large.
