子宫颈上皮内病变组织中人乳头瘤病毒载量与病变程度的关系
The relationship between the human papillomavirus viral load and the degree of pathology in the cervical intraepithelial neoplasia
【摘要】 目的通过检测各级别宫颈上皮内病变组织中不同亚型HPV病毒载量,探讨HPV型别病毒载量与宫颈癌发生的相关关系。方法选择病理诊断明确的CIN1、CIN2、CIN3与宫颈癌患者各20例,采用微切技术获取宫颈病变组织,以实时荧光PCR技术检测不同病变单位细胞(10 000个细胞)内的HPV亚型病毒载量。结果(1)宫颈病灶中HPV亚型感染率在CIN1、CIN2/3与宫颈癌分别为88%、100%和100%。HPV亚型在不同级别病变的分布不同,HPV16亚型在CIN3/宫颈癌的感染率(85%)明显高于CIN2(46%)与CIN1(47%),差异有统计学意义(P<0.05);HPV52亚型在CIN1、2、3中检出频率相当;HPV58亚型则在低度病变检出频率为高。(2)单位病变组织内HPV载量随病变程度增加而升高(P<0.05),感染率占前三位的HPV16、52、58亚型单位病变组织内的病毒载量在CIN2/3明显高于CIN1(P<0.05)。(3)HPV多重感染在CIN1、2、3与宫颈癌分别为47.1%、21.4%%、50%和26.7%,多重感染与病变的严重程度无相关性(r=-0.106,P>0.05)。结论宫颈病灶组织中HPV16、52、58亚型与宫颈病变密切相关,病变单位细胞内的HPV病毒载量随宫颈病变程度增加而升高。
【Abstract】 Objective To explore the relationship between HPV subtype viral load and the cervical cancer by investigating the different subtypes of HPV viral load in various grades of cervical lesions. Method The patients diagnosed by definite pathology were divided into CIN1, CIN2, CIN3, ICC four groups, 20 cases in each group. The cervical lesion embedded in paraffin were obtained by micro-cutting technique, and the HPV viral load of cell unit(10 000 cells) was detected with real-time fluorescence PCR technique. Analyzing viral load in different grades of HPV subtype, the pathogenesis of the viral load were explored. Result(1) The relevance ratio of HPV subtype in CIN1, CIN2-3 and cervical cancer were 88%, 100% and 100%, respectively. Different HPV subtypes had various distribution in the diverse grades of cervical lesions.The positive rate of HPV16 subtype in CIN3/cervical cancer(85%) was significantly higher than that in the CIN2(46%) and CIN1 47%(P<0.05). The frequency of HPV52 in CIN1, 2 and 3 was almost identical. HPV 58 subtype was found a high frequency in the low degree disease.(2) The HPV viral load of c ell unit within CLPE increased with the aggravation of cervical lesion(P<0.05). The top three positive frequencysubtype were HPV16, 52, 58. The unit viral load within the CLPE of high grade lesion was significantly higher than that of low grade lesions(P<0.05).(3) HPV multiple infection in CIN1, CIN2, CIN3 and cervical cancer were 47.1%, 21.4%%, 50% and 26.7% respectively.Multiple infections and the severity of the lesions did not show significant correlation. Conclusion HPV16, 52, 58 subtypes were closely related to cervical lesions. The HPV viral load in the disease cell increased with the aggravating of the cervical lesions degree.
