实验犬脑出血死亡后高场强MRI征象分析

【摘要】 目的探讨脑损伤出血死后血肿影像学信号随时间变化的规律。方法实验犬24只随机分为对照组和实验组,每组12只,对照组深度麻醉后处死,实验组模仿自然状态下机械性外力击打头部导致脑损伤出血死亡,两组分别于不同时点(死后1、2、3、6、12、36、60、84 h)行头部MRI检查,比较两组间T1WI与T2WI的信号变化。结果脑出血后血肿T1WI与T2WI信号随时间变化出现变化,正常对照组T1WI与T2WI信号随时间变化无明显变化,实验组异常信号发生率与对照组比较,差异有统计学意义(P<0.05)。结论脑损伤出血后血肿T1WI与T2WI信号随时间出现规律性变化,利用血肿影像学检查可推断脑损伤出血死后经过时间(即死亡时间),为法医学临床实际检案提供侦查线索和诉讼证据。

【Abstract】 Objective To investigate MRI features and changes with time after death because of brain injury. Methods 24 dogs were randomly divided into control group( 12 dogs) and Experimental group( 12dogs). The control group was killed after deep anesthesia and the Experimental group died of cerebral injury and hemorrhage after the heads were hit by imitating the mechanical forces. The dogs’ heads were examined by MRI at different points( 1 h,2 h,3 h,6 h,12 h,36 h,60 h and 84 h after death). The signal changes of T1 WI and T2 WI in the two groups were compared. Results The hematoma signal of T1 WI and T2 WI were changed with time after cerebral hemorrhage and the signal of T1 WI and T2 WI in the control group had no significant changes. The difference between the two groups was correlative( P < 0. 01).Conclusion The hematoma signal changes of T1 WI and T2 WI were regular with time after cerebral injury and hemorrhage. The hematoma imaging examination can be used to estimate the death time after cerebral injury and hemorrhage to provide clues and evidence for forensic clinical practice.
