冠脉内替罗非班联合血栓抽吸在急性ST段抬高心梗急诊介入的应用研究

【摘要】 目的探讨冠脉内替罗非班联合血栓抽吸在急性ST段抬高心梗急诊介入的应用。方法选择2012年5月至2013年12月收治的102例急性ST段抬高心梗患者,按治疗方式分作对照组与研究组,每组51例,对照组予以冠脉内替罗非班治疗,研究组基于对照组予以联合血栓抽吸治疗,比较两组心肌灌注情况,肌酸激酶同工酶(CK-MB),肌钙蛋白I(c Tn I),脑钠肽(BNP),血管内皮功能,和不良反应发生情况。结果研究组ST段恢复率、心肌MBG3级、TIMI血流3级率高于对照组(P<0.05)。研究组CK-MB、c Tn I、BNP低于对照组(P<0.05)。研究组血管内皮功能优于对照组(P<0.05)。两组不良反应率比较无差异(P>0.05)。结论冠脉内替罗非班联合血栓抽吸在急性ST段抬高心梗急诊介入中的效果肯定,能够改善心肌再灌注,缓解心肌损伤。

【Abstract】 Objective To research the application of intravascular Tirofiban Combined thrombus aspiration in acute ST segment elevation myocardial infarction with emergency intervention.Methods 102 cases of patients with acute ST segment elevation myocardial infarction from May 2012 to December 2013,according to treatment method divided into control group and research group,51 cases in each group,the control group was given intravenous tirofiban treatment,the research group was based on the control group treated with thrombus aspiration,myocardial perfusion,creatine kinase isoenzyme( CK-MB),troponin I( c Tn I),brain natriuretic peptide( BNP) and vascular endothelial function,and adverse reactions occur was compared between two groups. Results The ST segment recovery rate,myocardial MBG3,TIMI 3 blood flow rate of research group was higher than the control group( P<0.05). CK-MB,c Tn I,BNP of research group was lower than the control group( P < 0. 05). The endothelial function of research group was better than control group( P< 0.05). The adverse reaction rate was no difference between the two groups( P>0.05). Conclusion Positive effect of intravascular Tirofiban Combined thrombus aspiration in acute ST segment elevation myocardial infarction with emergency intervention,can improve myocardial reperfusion and reduce myocardial injury.
