杂合式血液净化对维持性血液透析患者β2微球蛋白水平及微炎症状态的影响

【摘要】 目的分析杂合式血液净化对维持性血液透析患者β2微球蛋白(β2-MG)水平及微炎症状态的影响。方法选取2015年4-10月重庆医科大学附属第一医院及重庆医科大学附属第一医院第一分院收治的维持性血液透析患者128例,将其分为单纯血液透析组与杂合式血液净化组,各64例。于治疗前及治疗6个月后,分别采集患者透析前血标本,测定β2-MG、超敏C反应蛋白(hs-CRP)与白细胞介素-6(IL-6)水平,并进行比较分析。结果治疗6个月后,杂合式血液净化组患者血β2-MG、hsCRP、IL-6水平较治疗前明显降低,差异均有统计学意义(P<0.05);而单纯血液透析组患者血β2-MG水平较治疗前降低,hsCRP、IL-6水平较治疗前升高,但差异均无统计学意义(P>0.05);治疗6个月后,杂合式血液净化组患者血β2-MG、hs-CRP、IL-6水平均低于单纯血液透析组,差异均有统计学意义(P<0.05)。结论杂合式血液净化可以更有效地清除透析患者血液中分子毒素,并减轻机体慢性微炎症状态。

【Abstract】 Objective To explore the effects of heterozygous blood purification on clearance ofβ2-microglohulin(β2-MG)and alleviation of microinflammatory state in patients on maintenance hemodialysis.Methods A total of 128 patients on maintenance hemodialysis for more than 6 months in the First Affiliated Hospital of Chongqing Medical University and the First Branch of the First Affiliated Hospital of Chongqing Medical University were enrolled and divided into the routine hemodialysis group and heterozygous blood purification group,64 cases in each group.The serum levels ofβ2-MG,high-sensitive C reactive protein(hs-CRP)and interleukin-6(IL-6)of patients in the two groups were measured before and 6 months after the initiation of hemodialysis.ResultsAfter 6 months of treatment,the serum levels ofβ2-MG,hs-CRP and IL-6 were decreased in heterozygous blood purification group,compared with those before treatment,the differences were statistically significant(P<0.05).In routine hemodialysis group,the serum level ofβ2-MG was decreased,and serum levels of hs-CRP and IL-6 were increased,,compared with those before treatment,no statistically significant difference was found(P>0.05).Moreover,after 6 months of treatment,the serum levels ofβ2-MG,hsCRP and IL-6 in heterozygous blood purification group were significantly lower than those in the routine hemodialysis group(P<0.05).Conclusion Heterozygous blood purification could efficiently remove middle molecule toxins from blood,and alleviate microinflammatory status in patients on maintenance hemodialysis.
