ER、HNF-1β及COX-2在子宫内膜异位相关性卵巢癌中的表达及临床意义

【摘要】 目的探究雌激素受体(ER)、肝细胞核因子-1β(HNF-1β)以及环氧化酶-2(COX-2)在不同类型子宫内膜异位相关性卵巢癌(EAOC)中的表达差异。方法选取2011年7月至2016年4月苏州大学附属第二医院收治的EAOC患者44例,其中子宫内膜样癌17例,透明细胞癌21例,卵巢浆液性癌6例。采用免疫组织化学法检测ER、HNF-1β及COX-2在不同类型EAOC中的表达差异,并采用Spearman秩相关分析探讨三者表达水平的相关性。结果ER在子宫内膜样癌患者中的阳性率为100.0%,显著高于透明细胞癌患者的9.5%和卵巢浆液性癌患者的0%(χ~2=4.305,P<0.01),而HNF-1β在透明细胞癌和卵巢浆液性癌患者中的阳性率分别为85.7%和100.0%,显著高于子宫内膜样癌患者的11.8%(χ~2=3.585,P<0.01),COX-2在子宫内膜样癌、透明细胞癌和卵巢浆液性癌患者中的阳性率分别76.5%、81.0%、83.3%,差异无统计学意义(χ~2=0.744,P=0.104)。ER和HNF-1β的表达水平呈负相关(r=-0.428,P<0.01),与COX-2的表达水平呈正相关(r=0.204,P=0.013)。结论 ER主要在子宫内膜样癌中表达,HNF-1β主要在透明细胞癌和卵巢浆液性癌中表达,ER的表达水平与HNF-1β及COX-2的表达水平具有一定的相关性。

【Abstract】 Objective To investigate the expression difference of estrogen receptor(ER),hepatocyte nuclear factor 1β(HNF-1β)and epoxy-2(COX-2)in different types of endometriosis-associated ovarian cancer(EAOC).Methods Forty-four patients with EAOC treated in our hospital from July 2011 to April 2016 were selected,including 17 cases of endometrioid carcinoma,21 cases of clear cell carcinoma and 6cases of ovarian serous carcinoma.The expression of ER,HNF-1βand COX-2in different types of EAOC were detected by immunohistochemistry.The correlation among expression levels of ER,HNF-1βand COX-2was investigated by adopting Spearman rank correlation analysis.Results The positive rate of ER in endometrioid carcinoma was 100.0%,which was significantly higher than 9.5%in the patients with clear cell carcinoma and 0%in the patients with ovarian serous carcinoma(χ~2=4.305,P<0.01),and the positive rate of HNF-1βin the patients with clear cell carcinoma and ovarian serous carcinoma were85.7% and 100.0% respectively,which was significantly higher than 11.8% in the patients with endometrioid carcinoma(χ~2=3.585,P<0.01),the positive rates of COX-2in the patients with endometrioid carcinoma,clear cell carcinoma and ovarian serous carcinoma were 76.5%,81.0% and 83.3%,respectively,and the difference was not statistically significant(χ~2= 0.744,P=0.104).The correlation analysis showed that ER was negatively correlated with HNF-1βexpression level(r=-0.428,P<0.01)and positively correlated with COX-2expression level(r=0.204,P=0.013).Conclusion ER is mainly expressed in endometrioid carcinoma.HNF-1βis mainly expressed in clear cell carcinoma and ovarian serous carcinoma.The expression level of ER had a certain correlation to the expression of HNF-1βand COX-2,which may be closely related to EAOC progression.
