不同血脂水平人群中心肌梗死事件的危险因素分析
Risk factors of central myocardial infarction in patients with different lipid levels
【摘要】 目的探讨在不同的空腹三酰甘油(TG)水平的人群中心肌梗死(MI)的发生情况及其高危影响因素。方法以2006年6月至2007年10月开滦煤矿集团健康体检的在职和离退休职工为研究对象(n=100 271),根据不同TG水平,将研究人群分为:TG1组(0.01<TG≤0.81)mmol/L,TG2组(0.81<TG≤1.19)mmol/L,TG3组(1.19<TG≤1.46)mmol/L,TG4组(1.46<TG≤2.16)mmol/L和TG5组(2.16<TG≤19.95)mmol/L,随访7年。采用Logistic回归法进行多因素分析,采用Cox比例风险回归法分析不同水平TG对终点事件的影响。结果多因素Logistic回归分析显示,高TG水平人群中,年龄大于或等于65岁,BMI>25kg/m2,空腹血糖(FBG)≥6.1mmol/L,高密度脂蛋白胆固醇(HDL-C)<1.5mmol/L等均为发生MI的危险因素(P<0.05)。Cox比例风险回归模型显示,校正性别、年龄等因素后,随着TG水平的升高,发生MI的相对危险亦增加。结论在研究的开滦人群中,空腹TG水平升高者MI发生风险显著增加,而年龄、BMI、FBG和HDL-C是空腹TG水平升高人群发生MI的危险因素。
【Abstract】 Objective To investigate the incidence of miocardial infarction and risk factors in patients with different levels TG.Methods From June 2006 to October 2007,Kailuan coal mine group conducted an on-the-job and retired workers were took physical examination in Kailuan area,and their results were used in our study(n=100 271).According to different levels of TG,all cases were divided into five groups(TG1-5):TG1group(0.01<TG≤0.81)mmol/L,TG2group(0.81<TG≤1.19)mmol/L,TG3group(1.19<TG≤1.46)mmol/L,TG4group(1.46<TG≤2.16)mmol/L and TG5group(2.16<TG≤19.95)mmol/L.All patients were followed up for 7years.Multivariate analysis was performed by Logistic regression method.Cox proportional hazards regression was used to analyze the influence of different levels of TG on the end point.Results Multivariate Logistic regression analysis showed that in high TG level population groups,the indicators including aged ≥65years,BMI>25kg/m2,FBG≥6.1mmol/L,HDL-C<1.5mmol/L were all the risk factors for myocardial infarction(P<0.05).Cox proportional hazards regression model showed that after adjustment for sex,age and other factors,with the increase in TG levels.Conclusion In study of Kailuan crowd,the increased fasting triglycerides increase the risk of myocardial infarction,and the risk factors for myocardial infarction are age,BMI,FBG and HDL-C.
