参芪扶正注射液治疗慢性心力衰竭随机对照试验的Meta分析
Shenqi Fuzheng Injection for treatment of Chronic Heart Failure:a Meta-analysis on Randomized Controlled Trials
【摘要】 目的运用Meta分析对参芪扶正注射液治疗慢性心力衰竭的临床疗效进行评价。方法计算机检索中国期刊全文数据库(CNKI)、中国生物医学全文数据库(CBM)、中文科技期刊全文数据库(VIP)、中国重要会议论文全文数据库、美国国立医学图书馆(PubMed)等数据库,收集参芪扶正注射液治疗慢性心力衰竭近3年的文献,按照研究的文献纳入标准和文献质量评价纳入文献,提取数据后,应用RevMan 5.3进行Meta分析。结果最终纳入文献10篇,Meta分析提示临床有效率、SV具有同质性(I~2<50%),合并效应量具有统计学意义(P<0.01);脑利钠肽(BNP)、左心室射血分数(LVEF)、心输出量(CO)和每搏输出量(SV)具有异质性(I 2>50%),合并效应量具有统计学意义(P<0.01);左心室舒张末期内径(LVED)合并效应量无统计学意义(P=0.05)。结论西医常规用药联合参芪扶正注射液可以提高治疗心力衰竭的临床疗效,能够改善心功能,纳入研究质量较低,设计不够严谨仍然是不可忽视的因素,此结论仍需多中心、大样本、高质量研究证实。
【Abstract】 Objective This study was aimed to use meta analysis to assess the clinical therapeutic effects of Shenqi Fuzheng injection(SFI)in the treatment of chronic heart failure(CHF).Methods Using the computer to retrieve the CBM,CNKI,VIP and PubMed database and collecting the article of treatment for CHF by SFI for 3 years recently.According to the study of the inclusion criteria and quality of literature evaluation to screen and analyze literature by using RevMan5.3.Results We eventually screen10 references.Meta-analysis showed that there was homogeneity in therapeutic effect and SV(I 2<50%)and there was statistical significance on effect size(P <0.01).There was heterogeneity in B-type natriuretic peptide(BNP),left ventricular ejection fraction(LVEF),left ventricular diastolic diameter(LVED)and cardiac output(CO)(I~2>50%)and there was statistical significance on effect size(P <0.01)but except LVED(P =0.05).Conclusion There was more better clinical therapeutic effect for CHF treated with SFI based on treating with the symptomatic treatment,and the cardiac function could be improved.Limited by the quality of the inclusive references,more rigorous scientific research design,large sample,multicenter study need to be further analyzed.
