槟榔对健康人胃电图及胃动素、促肾上腺皮质激素释放激素的影响

【摘要】 目的探讨槟榔颗粒促进胃动力的作用机理。方法 40例健康志愿者随机分成槟榔组、盐水对照组、进餐组、槟榔+进餐组、盐水+进餐组,应用多导胃电图记录仪检测健康志愿者胃电图4 h。观察胃体、胃窦不同区域的胃电指数。采用ELISA法检测服用槟榔前、后脑肠肽(BGP)激素水平的变化。结果 (1)与盐水对照组比较,槟榔组服药后15 min~2 h胃体及胃窦部的胃电主功率比显著增强(P<0.05),主功率比的高峰值在口服槟榔后的15~30 min时段内;(2)槟榔+进餐组、盐水+进餐组、进餐组餐后2 h内主功率比均大于1,提示餐后主功率较静息期显著增高。槟榔+进餐组胃体中下部和胃窦部主功率比显著高于盐水+进餐组和进餐组(P<0.05)。盐水+进餐组与进餐组比较主功率比差异无统计学意义(P>0.05)。槟榔+进餐组餐后的主频率在胃体中下部和胃窦部显著增高(P<0.05);正常慢波百分比有增高趋势,不稳定系数有下降趋势。(3)激素水平:在服用槟榔后与空腹静息期比较血浆胃动素(MTL)含量显著增高(P<0.05),促肾上腺皮质激素释放激素(CRH)显著降低(P<0.05);与盐水+进餐组比较,槟榔+进餐组服药后及进餐后MTL显著增高,CRH显著降低(P<0.05)。结论槟榔能够通过增强胃电活动促进胃动力,其作用机理可能是通过BGP激素MTL、CRH介导。

【Abstract】 Objective To investigate the therapeutic mechanism of Semen Arecae(SA) granule on improving the gastric motility. Methods Forty healthy people were randomly divided into the SA group, saline group, meal group,SA + meal group, and saline + meal group. The electrogastrogram of the studying objects was recorded with the multi- guide gastric recorder for 4 hours. The outcomes covered gastric electrical indexes of the gastric body and gastric antrum, as well as brain gut peptide(BGP) levels detected by using ELISA method. Results(1)Compared to the saline group, gastric electric main power ratio of the gastric body and gastric antrum in SA + meal group was significantly enhanced 15 min- 2 h after oral administration(P < 0.05),and the peak value of the main power ratio was present 15- 30 minutes after administration.(2)The main power ratio of SA + meal group,saline + meal group,and meal group was over 1 within 2 h after administration,suggesting that postprandial main power was significantly higher than the main power at resting period. The main power of the gastric antrum and the middle and lower part of the gastric body of SA + meal group was significantly higher that of the saline + meal group(P < 0.05). There was no significant difference of the main power ratio between saline + meal group and meal group. The postprandial main frequency of the gastric antrum and the middle and lower part of the gastric body was also increased significantly(P <0.05),and the percentage of normal slow wave showed an increasing trend and unstable coefficient had a decreasing trend.(3)The postprandial motilin(MTL)level was higher while corticotropin- releasing hormone(CRH)was lower than the levels at resting period(P < 0.05). Compared with the saline group,MTL was significantly increased and CRH was significantly reduced in SA group(P < 0.05). After meal,the plasma MTL content of SA group was significantly higher and CRH was significantly lower than those of the saline group(P < 0.05). Conclusion SA is effective on promoting gastric motility by enhancing gastric electrical activity,which may be mediated by BGP hormones MTL and CRH.
