以病机为核心的中医临床课程案例式教学改革研究探讨

【摘要】 突出病机的主体地位,以中医原创性理论和思维模式作为教学的主线,应是贯彻于高等中医药院校中医教学过程中的基本原则。案例式教学法与中医"审证求因、审因论治"的思维模式有着形式上的相似之处,即两者都是首先抓住一个临床问题(Case或主证),再围绕这个问题展开思维。将案例式教学模式作为病机构建过程的承载体是践行中医"在继承基础上的发展"这一宗旨的创新。师承是中医的传统教学模式,以教学班为单位的床边教学是其现代的延伸,属于模拟教学范畴;其主要的实现形式应该是以病机为核心的案例式教学,这是当前高等中医药院校实施"5+3"教学改革的重大选项。

【Abstract】 It should be carried out as a basic principle to highlight the dominant role of pathogenesis and theoriginal TCM theory and thinking mode in the whole teaching process. There is a similarity between the case-basedteaching method and the TCM thinking mode of"identifying causes from different syndromes and decidingtherapeutic methods from different causes", which both grasp a clinical problem first, followed by thinking aroundthe issue. It is an innovation of"developing on the basis of inheritance"to make the case-based teaching as thesupporting body of the thinking mode of"identifying causes from different syndromes and deciding therapeuticmethods from different causes". The succession of teachings from a master to his disciples is the traditional teachingmode of TCM while the class of the bedside teaching is its modern extension, which falls into the category ofsimulation teaching and can be implemented by case-based teaching with the core of pathogenesis. Therefore it isthe weighty option of the reform of"5+3"teaching mode in TCM colleges and universities.
