两种剂量阿托伐他汀短期治疗对急性冠脉综合征患者hs-CRP及NT-proBNP水平的影响

【摘要】 目的:研究大剂量(40 mg/d)和常规剂量(20 mg/d)阿托伐他汀治疗2周后对急性冠脉综合征(ACS)患者血浆超敏C反应蛋白(hs-CRP)及血浆N-末端脑钠肽前体(NT-proBNP)水平的影响。方法:选取2012年10月-2016年3月本院收治的90例ACS患者作为研究对象,随机分为常规剂量组(阿托伐他汀20 mg/d)和大剂量组(阿托伐他汀40 mg/d),测定治疗前及治疗2周后低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、hs-CRP及NT-proBNP水平。结果:两组治疗2周后HDL-C和TG变化不明显,LDL-C和TC均下降(P<0.05),大剂量组下降更明显;常规剂量组和大剂量组hs-CRP和NT-proBNP水平均有下降,大剂量组下降有显著性(P<0.01);相关性分析显示,治疗后hs-CRP及NT-proBNP与血脂水平无显著相关性。结论:短期大剂量阿托伐他汀治疗可以使血脂、hs-CRP及NT-proBNP水平显著下降,减少炎症反应,稳定动脉粥样斑块,减少冠脉事件的发生。

【Abstract】 Objective:To study the effects of high dose Atorvastatin(40 mg/d) and conventional dose Atorvastatin(20 mg/d) on plasma levels of hypersensitive C-reactive protein(hs-CRP) and N-Terminal fragment of the prohormone brain-type natriuretic peptide(NT-proBNP) in patients with acute coronary syndrome(ACS) after two weeks treatment.Method:From October 2012 to March 2016,90 cases of ACS in our hospital were selected as the research objects,they were randomly divided into the conventional dose group(Atorvastatin 20 mg/d) and high dose group(Atorvastatin 40 mg/d).The levels of lower density lipid cholesterol(LDL-C),total cholesterol(TC),triglycerides(TG),higher density lipid cholesterol(HDL-C),hs-CRP and NTpro BNP of two groups were measured before and after two weeks treatment.Result:Treatment in the two groups after 2 weeks of HDL-C and TG change is not obvious,TC and LDL-C decreased(P<0.05),high dose group decreased more significantly.The hs CRP and NT pro BNP of conventional dose group and high dose group were decreased,large dose group decreased significantly(P<0.01).Correlation analysis showed that there were no significant correlation between hs-CRP,NT-proBNP and blood lipid levels after treatment(P>0.05).Conclusion:Short-term high-dose Atorvastatin may reduce serum lipid,plasma hs-CRP and NT-proBNP levels significantly,reduce the inflammatory reaction,stabilize the atherosclerotic plaque and reduce the incidence of coronary events.
