【摘要】 目的:了解社区患有心血管疾病的老人共病抑郁的情况,初步分析共病相关因素。方法:选取河北省4市区60岁以上社区老年人3176例,采集一般人口学资料,调查心血管疾病如高血压病、高脂血症、冠心病、心律失常和心绞痛的患病情况,采用流调用抑郁自评量表(CES-D)评估抑郁症状,采用匹兹堡睡眠质量指数量表(PQSI)、日常生活能力量表(ADL)评定睡眠状况及日常生活能力。结果:参与调查的3176例老人中,1889例患有心血管疾病,179例(9.5%)共病抑郁。高血压病伴发抑郁的有137例(9.6%),高血脂症伴发抑郁的有69例(9.3%),冠心病伴发抑郁的有72例(12.8%),心绞痛伴发抑郁的有48例(17.3%),心律失常伴发抑郁的有36例(12.1%)。多因素logistic回归分析显示,女性(OR=1.62)、日常生活能力差(OR=4.88)、睡眠障碍史(OR=3.92)、体检不规律(OR=1.20)都会增加共病的发生。参加社会和娱乐活动(OR=0.65)的老人共病率低。结论:心血管疾病共病抑郁的发生可能与女性、日常生活能力差、睡眠障碍史、体检不规律有关。更多还原

【Abstract】 Objectives:To investigate the prevalence of comorbid depression in community elderly with cardiovascular disease(CVDs) and the risk factors.Methods:The cross-sectional population-based study was conducted in Hebei province between January 2010 and January 2011.Totally 1889 elderly with CVDs involved in the study were assessed for the comorbidity of CVDs and depression.A questionnaire was constructed to collect socio-demographic data and the Pittsburgh Sleep Quality Index Scale(PQSI) and Activities of Daily Living Scale(ADL) were used to assess their sleep status and activities of daily living.Results:In 1889 elderly with cardiovascular disease,179 cases(9.5%) were with comorbid depression.Multivariable logistic regression analysis indicated that women(OR= 1.62),ADL scores of >26(OR=4.88),PQSI scores of >7(OR = 3.92),irregular physical activity(OR= 1.20) were associated with increased occurrence of comorbidity.Participating in social and recreational activities(OR=0.65) could reduce the incidence of comorbidity.Conclusion:It suggests that female gender,poor daily living ability,sleep disturbance and irregular physical examination may contribute to occurrence of cardiovascular cocomorbid depression.更多还原
