Ag85A、Ag85B、16kDa和38kDa用于结核病血清学诊断的评价

【摘要】 目的对Ag85A、Ag85B、16k Da和38k Da 4种结核分枝杆菌抗原和几种抗原的组合进行诊断效能评价,分析其应用价值。方法采用酶联免疫吸附试验,包被抗原使用Ag85A、Ag85B、16k Da和38k Da 4种原核系统表达的重组结核分枝杆菌抗原,检测54份结核病患者血清和93份健康者血清,并利用受试者工作特征曲线法计算不同抗原的最佳临界值(cut-off值),计算单种抗原和抗原组合的阳性率,对其诊断价值进行综合评价。结果 4种抗原的最佳包被浓度分别为3.05 mg/ml、4.50 mg/ml、2.45 mg/ml和7.50 mg/ml,最佳血清稀释度分别为1∶200、1∶400、1∶200、1∶200;11种抗原组合中Ag85B+16k Da+38k Da的组合诊断效能效果最好,灵敏度为70.4%,特异度为80.0%,诊断效率为76.2%,Youden指数为0.504。结论抗原组合Ag85B+16k Da+38k Da取得了较高的检测灵敏度,相比单种抗原更具有诊断价值。

【Abstract】 Objective To evaluate the efficacy of Mycobacterium tuberculosis antigens( Ag85 A,Ag85B,16 k Da,38 k Da) and their combinations,and to analyze the diagnostic value. Methods Enzyme- linked immunosorbent assay( ELISA),coating with prokaryotic recombination Mycobacterium tuberculosis antigens Ag85 A,Ag85B,16 k Da,38 k Da respectively,was adopted to test 54 serums from TB patients and 93 serums from healthy people. Receiver operating characteristic( ROC) was drawn to get the cut- off value,with which the sensitivity of 4 antigens and different combinations could be calculated. And the comprehensive evaluation was conducted on the diagnostic value. Results It showed that the best coating concentrations of 4 antigens were 3. 05 mg / ml,4. 50 mg / ml,2. 45 mg / ml and 7. 50 mg / ml respectively. The best dilutions of serums were 1 ∶ 200,1∶400,1∶200,1∶200. Ag85 B + 16 k Da + 38 k Da had the best diagnostic efficacy of the 11 combinations,of which the sensitivity,specificity,diagnostic efficiency and Youden index were 70. 4%,80. 0%,76. 2% and 0. 504. Conclusion The combination Ag85 B + 16 k Da + 38 k Da has higher detection sensitivity and better performance for TB diagnosis than single antigen.
