【摘要】 目的观察雷珠单抗注射液(RZB)玻璃体腔内注射联合黄斑格栅样光凝(MGP)治疗糖尿病黄斑水肿(DME)患者的临床效果。方法 85例DME患者,随机分为对照组(42例)和观察组(43例),对照组患者予以MGP治疗,观察组患者在此基础上联合RZB玻璃体腔内注射,比较两组疗效及并发症。结果两组患者治疗后的黄斑区域平均厚度(CAT)、黄斑中心凹厚度(CST)、最佳矫正视力(BCVA)水平均较治疗前下降,且观察组下降幅度大于对照组(P<0.05)。对照组患者的眼压升高、结膜出血、前房反应的例数分别为2、3、4例,总并发症发生率为21.43%;观察组患者的眼压升高、结膜出血、前房反应的例数分别为0、1、1例,总并发症发生率为4.65%,对照组的总并发症发生率高于观察组(P<0.05)。结论以玻璃体腔内注射RZB联合MGP治疗能有效改善DME患者黄斑情况,提高视力,且并发症少,具有临床推广价值。更多还原

【Abstract】 Objective To observe clinical effect by intravitreal ranibizumab(RZB) injection combined with macula lutea grid pattern photocoagulation(MGP) in the treatment of diabetic macular edema(DME) patients. Methods A total of 85 DME patients were randomly divided into control group(42 cases) and observation group(43 cases). The control group received MGP treatment, and the observation group received additional intravitreal RZB injection for combined treatment. Curative effects and complications were compared between the two groups. Results Both groups had lower mean macula lutea thickness(CAT), macula lutea central foveal thickness(CST) and best corrected visual acuity(BCVA) after treatment than those before treatment, and the observation group had higher descend range than the control group(P<0.05). The control group had 2 cases with increasing intraocular pressure, 3 conjunctival hemorrhage cases and 4 cases with anterior chamber reactions, along with total incidence as 21.43%. The observation group had 0 case with increasing intraocular pressure, 1 conjunctival hemorrhage case and 1 case with anterior chamber reactions, along with total incidence as 4.65%. The control group had higher total incidence of complications than the observation group(P<0.05). Conclusion Combination of intravitreal RZB injection and MGP can effectively improve macula lutea condition in DME patients, along with enhanced visual acuity and few complications.
