【摘要】 目的探讨改良处理远端疝囊预防巨大腹股沟斜疝术后阴囊血肿、血清肿的有效性及优越性。方法回顾性分析2007年1月至2014年10月,广东省梅州市大埔人民医院收治并施行疝修补术的187例巨大腹股沟斜疝患者的临床资料。根据对疝囊处理方式不同分为4组。A组86例,不放置胶管引流,给予远端疝囊残腔作连续螺旋内荷包缝扎改良处理;B组29例,不放置胶管引流,并且不作远端疝囊残腔连续螺旋内荷包缝扎改良处理;C组32例,给予远端疝囊全剥除,不作连续螺旋内荷包缝扎改良处理;D组40例,给予放置胶管引流,不作远端疝囊残腔连续螺旋内荷包缝扎改良处理。比较其手术时间、住院时间、术后阴囊血肿、血清肿、积液发生率及复发率。结果 187例巨大腹股沟斜疝患者疝修补术均顺利完成,4组手术时间和复发率比较,差异无统计学意义;住院时间、阴囊血肿、血清肿、积液发生率比较,差异有统计学意义。结论给予远端疝囊残腔作连续螺旋内荷包缝扎改良处理,能安全有效预防巨大腹股沟斜疝术后阴囊血肿、血清肿并发症的发生,特别适用于多发巨大腹股沟斜疝患者的山区基层医院推广应用。更多还原

【Abstract】 Objective To investigate the effect of modified distal hernia treatment for prevention of scrotal hematoma and seroma after giant inguinal hernia repair. Methods From January 2007 to October2014,the patients with giant inguinal hernia were received hernia repair with distal hernia sac exclusion in The Da-Pu people’s Hospital of Meizhou,which were divided into four groups according to the treatment of the hernia sac. Group A: 86 cases,didn’t place the drainage tube,given distal hernia sac string suture with modified treatment. Group B: 29 cases,didn’t place the drainage tube and didn’t received distal hernia sac string suture. 32 cases in group C. 40 cases in group D didn’ t received distal hernia sac string suture but with a drainage tube placement. The clinical data were retrospectively analyzed. The operative time,postoperative hospital stay,postoperative scrotal hematoma,seroma,effusion and recurrence rate between four groups were compared. Results 187 cases of huge inguinal hernia were performed successfully. The operation time and recurrence rate have no significant difference,but the hospitalization time,scrotal hematoma,seroma and effusion were showed significant differences. Conclusion Distal hernia sac with the continuous spiral string suture is safe and effective for the prevention of scrotal hematoma and seroma after the repair of huge indirect inguinal hernia,especially for multiple type huge inguinal hernia patients mountain of popularization and application in primary hospitals.更多还原
