【摘要】 目的比较完全腹膜外疝修补术(TEP)术式和平片无张力疝修补术李金斯坦(Lichtenstein)术式的临床疗效。方法 2011年1月至2012年12月,首都医科大学附属北京世纪坛医院采用前瞻性随机对照研究方法,应用TEP术式和李金斯坦术式治疗120例初发腹股沟疝患者,观察术中指标(手术时间、术中出血量)、术后近期指标(下床活动时间、住院时间)、并发症(尿潴留、切口感染、切口感觉异常、切口疼痛、射精疼痛、血清肿、阴囊水肿)、术后远期指标(如复发率)等。结果二组患者手术时间分别为(39±5)min、(41±5)min,术中出血量分别为(8.5±2.1)ml、(8.8±2.5)ml,下床活动时间分别为(0.9±0.2)d、(1.0±0.3)d,住院时间分别为(4.3±0.3)d、(4.7±0.3)d,复发率分别为0、3.3%,组间比较,差异无统计学意义(P>0.05)。二组患者并发症总发生率分别为13.3%、28.3%,组间比较,差异有统计学意义(P<0.05)。结论 TEP术式和李金斯坦术式均是治疗腹股沟疝的安全有效的手术方法,只要谨慎操作和掌握技巧,可以取得满意的治疗效果。更多还原

【Abstract】 Objective To compare the effect of two different tension-free hernia repair operations:TEP and Lichtenstein tension-free inguinal hernia repair. Methods Between January 2011 and December2012,a prospective randomized controlled trial in Beijing Shijitan was conducted on 120 cases of primary inguinal hernia hospital who were randomized assigned into two groups,open TEP group( n = 60) and Lichtenstein group( n = 60). Intraoperative data( operative time,blood loss),short-term postoperative data( ambulation time,hospital stay),complications( urinary retention,wound infection,paresthesia of the incision,incision pain,painful ejaculation,seroma and scrotal edema),and long-term data such as recurrence rate were compared. Results The operative time of the two groups were( 39 ± 5) minutes and( 41 ± 5) minutes,blood loss were( 8. 5 ± 2. 1) ml and( 8. 8 ± 2. 5) ml,ambulation time were( 0. 9 ±0. 2) days and( 1. 0 ± 0. 3) days,length of hospital stay were( 4. 3 ± 0. 3) days and( 4. 7 ± 0. 3) days.Recurrence rates were 0% and 3. 3%( 2 /60). There were no statistically significant difference between the two groups( P > 0. 05). The overall complication rate of the two groups were 13. 3%( 8 /60),28. 3%( 17 /60),respectively. This difference was statistically significant between the two groups( P < 0. 05).Conclusion Both open TEP and Lichtenstein repair are safe and effective for inguinal hernia. As long as the surgeons were cautious and skillful,satisfactory therapeutic effect can be achieved.更多还原
