【摘要】 目的比较胃炎组织和胃癌组织中幽门螺杆菌感染和细胞周期蛋白D1（CyclinD1）蛋白表达,探索两者与胃癌发生发展的相关性。方法胃黏膜病变标本80例,其中慢性非萎缩性胃炎20例（CAG组）,萎缩性胃炎伴肠化生或不典型增生20例（NAG组）,胃癌40例（GC组）。采用¹³C呼气试验和姬姆萨染色法检测幽门螺旋杆菌感染,采用免疫组织化学法标记CyclinD1,检测其在不同病变胃黏膜组织中的蛋白表达情况,结合临床病理特征进行统计学分析。结果 NAG组、CAG组和GC组感染率差异有显著性。幽门螺旋杆菌感染与胃癌组织浸润深度有关,但与年龄、性别、肿瘤大小、临床分期、分化程度无关（P>0.05）。与CAG组和NAG组相比,GC组中CyclinD1阳性率显著增加;CyclinD1蛋白的表达水平与胃癌组织临床分期、浸润深度、分化程度有关（P<0.05）,而与年龄、性别和肿瘤大小无关（P>0.05）;胃癌组织中幽门螺旋杆菌感染与CyclinD1蛋白表达之间呈正相关（r=0.274,P<0.05）。结论 GC组中幽门螺旋杆菌感染率和CyclinD1阳性表达率均明显高于NAG和CAG两组。幽门螺旋杆菌感染和CyclinD1蛋白阳性表达率均与胃癌的发生发展相互关联,并且两者之间存在正相关的联系。更多还原

【Abstract】 Objective Through analysis of Helicobacter pylori（ H.pylori） infection and Cyclin D1 expression in atrophic gastritis tissues and gastric cancer tissues,we would like to investigate their correlation with the progress of gastric cancer. Method¹³C-urea breath test and Giemsa staining were used to detect H. pylori infection. Immunohistochemical staining was applied to detect the expression of Cyclin D1 and statistical analysis was utilized to dig out their correlation with the clinical pathological characteristics of gastric cancer.Result The infection rate of H.pylori in chronic atrophic gastritis group（ CAG） and atrophic gastritis with intestinal metaplasia or atypical hyperplasia group（ NAG）was significantly different from that in gastric cancer group（ GC）（ P<0.01）.At the same time,the infection rate of H.pylori was related to the depth of infiltration range（ P < 0. 01）,while it was not related with age,sex,gastric cancer size,clinical stages,and cell differentiation grade（ P>0.05）. Compared with that in CAG group and NAG group,the positive rate of Cyclin D1 was significantly increased in GC group.The expression of Cyclin D1 was related to clinical stages,depth of infiltration range and cell differentiation grade（ P<0.05）,but was not related with the age,sex,and gastric cancer size（ P>0.05）.H.pylori infection and the expression of Cyclin D1 in gastric cancer tissue was positive related（ r = 0.274,P<0.05）. Conclusion The infection rate of H.pylori and positive rate of Cyclin D1 in GC was significantly higher than that in CAG and NAG.Both of H.pylori infection rates and Cyclin D1 expression were involved in the development of gastric cancer.更多还原
