不同分子亚型的乳腺癌组织中神经生长因子及其受体的表达及意义
To explore the expression of nerve growth factor and its receptors in different molecular subtype of breast cancer
【摘要】 目的探讨乳腺癌组织中神经生长因子(NGF)以及其高亲和力受体(Trk A)、低亲和力受体(p75NTR)的差异表达与患者临床病理参数及分子亚型间的关系。方法运用免疫组织化学的方法检测乳腺癌组织(n=80)NGF、Trk A、p75NTR的表达,并选取良性乳腺疾病组织(n=30)和癌旁正常组织(n=30)作为对照。根据雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(Her-2)及增殖细胞核抗原67(nuclear protein Ki-67)的表达将其分为四个分子亚型。最后,分析NGF、Trk A、p75NTR的表达与乳腺癌患者临床病理参数的关系,并分析各亚型中NGF、Trk A的表达差异。结果癌组织中NGF、Trk A的表达显著高于两对照组(χ2=26.32、48.16,P<0.01),其表达分别与淋巴结转移及p TNM分期相关(χ2=5.61、5.12、3.95、5.32,P<0.05);而p75NTR的表达显著低于两对照组(χ2=56.47,P<0.01),其表达与临床病理各参数间无显著关联(P>0.05)。Luminal A型中Trk A的异常高表达率最低(48%)(χ2=8.56,P<0.05)。结论 NGF及其受体在乳腺癌组织中的差异表达与患者预后相关,不同分子亚型中的Trk A的表达有显著差异,检测其三者表达可作为评估高转移、复发风险乳腺癌患者的潜在指标。
【Abstract】 Objective To discuss the expression of nerve growth factor(NGF), its high affinity receptor(Trk A) and its low affinity receptors(p75NTR) in breast cancer tissues of different subtypes and to investigate the relationship between their expression and clinical significance. Methods The protein level of NGF, Trk A and p75 NTR of breast cancer tissues(n=80) were detected by immunohistochemical method, and the benign breast disease tissues(n=30), non-carcinoma tissues(n=30) were detected as control groups. Also, the breast cancers were divided into four molecular subtypes according to the expression of ER, PR, Her-2 and Ki-67. Then, the protein level of NGF, Trk A and p75 NTR in different subtypes was analyzed and the relationship between the protein level of NGF, Trk A in breast cancer tissues and clinical significance were assessed. Results The expression level of NGF and Trk A were significantly increased than two control groups(χ2=26.32, 48.16, P<0.01); their expression were associated with lymph node metastases and p TNM stage(χ2=5.61, 5.12, 3.95, 5.32, P<0.05); The expression level of p75 NTR was significantly reduced than two control groups(χ2=56.47, P<0.01), but the abnormal expression in three groups between clinical significance had no statistical significance(P>0.05). The abnormal protein level of Trk A in Luminal A were lowest(48%, χ2=8.56, P<0.05). Conclusion The expression of NGF and its receptors were significant related to the prognosis of breast cancer patients, and the protein level of Trk A in four molecular subtypes of breast cancer tissues was remarkable difference. NGF and NGFR may be important indicators to evaluate the high risk of lymph node metastases and patients’ recurrence.
