【摘要】 目的:比较三种机用镍钛锉WaveOne等对老年人牙髓根尖周病的磨牙进行一次性根管治疗术的临床疗效分析。方法:按照门诊就诊顺序随机将167例老年患者180颗诊断为牙髓坏死、急慢性牙髓炎或慢性根尖周炎的磨牙平均分为3组,每组60颗,分别为WaveOne组,Protaper组和K锉组。三组病例均采用一次性完成根管治疗,其中机用WaveOne组和Protaper组采用冠向下法进行预备根管,而K锉组采用逐步后退法。WaveOne组,Protaper组采用大锥度牙胶尖和AH plus根充糊剂冷侧压法充填根管;K锉组采用相应的标准牙胶尖和AH plus根充糊剂冷侧压法充填根,比较不同器械和不同根管预备方法完成根管治疗所需的时间以及患者术后24h、48h、72h及7天的疼痛反应。结果:三组间根管预备时间的差异有统计学意义(P<0.001),其中WaveOne组用时最短(2.51±0.60min),K锉组用时最长(9.53±0.65min);WaveOne组根管充填恰填率为93.3%,Protaper组恰填率为90.0%,K锉组恰填率为83.3%;术后24h及48h,三组间疼痛值差异有统计学意义(P<0.05);而72h及7天三组间差异无统计学意义(P>0.05);但四个时间点,WaveOne组与Protaper组相比均无统计学差异(P>0.05)。结论:老年患者牙髓根尖周病的磨牙采用WaveOne镍钛器械进行一次性根管治疗术,省时高效,减少复诊次数,增加患者依从性,值得推广。更多还原

【Abstract】 Objective:Comparing three kinds of nickle titanium file instrument for the elderly molar with endodontic disease to analyze the clinical curative effect of one-off root canal therapy.Methods:In accordance with the order of outpatients,167 cases of elderly patients with 180 teeth diagnosed as pulp necrosis,acute and chronic pulpitis or chronic periapical periodontitis were divided into 3 groups.Each group had 60 teeth,respectively for the group WaveOne and ProTaper group and K file group.Three groups of cases used a one-off root canal treatment,one machine with WaveOne and Protaper groups using crown down to prepare the root canal,and K file group using step-back method.WaveOne groups,Protaper group used large taper gutta percha and AH plus sealer filling root cannal with cold lateral compaction;K file group used the corresponding standard gutta percha and AH plus sealer filling filling root cannal with cold lateral compaction.Comparison of different instruments and different root canal preparation methods to complete the root canal therapy and the time required for patients with postoperative 24 h,48h,72 h and 7 days of pain response.Results:There were statistically significant differences in root canal preparation time among the three groups(P<0.001),the WaveOne group for the shortest time(2.51 + 0.60 min),K file group with the longest(9.53+0.65min);WaveOne group of root canal filling just fill rate was 93.3%,the Protaper group just to fill rate was 90.0%,the K file group just to fill rate was 83.3%.24 h and 48 h after surgery,pain value difference between three groups was statistically significant(P<0.05);The 72 h and 7 days there was no statistically significant difference among the three groups(P>0.05);But four time points,compared with group Protaper WaveOne group were no statistical differences(P>0.05).Conclusions:Elderly patients with dental pulp periapical disease of molar with WaveOne nickel titanium equipment disposable root canal treatment,time saving and efficient,reduce the number of follow-up,increase patient compliance,is worth promoting.更多还原
