2型糖尿病患者血清chemerin水平与骨密度相关性研究
The relationship between serum chemerin level and bone mineral density in T2DM patients
【摘要】 目的探讨2型糖尿病患者血清chemerin水平与骨密度及相关因素的关系。方法选取2012年1月2014年12月本院内分泌科收治的2型糖尿病住院患者,行双能X线骨密度检查,根据WHO骨质疏松诊断标准分为2型糖尿病骨量正常组42例、2型糖尿病骨量减少组50例、2型糖尿病骨质疏松(OP)组58例。记录其身高、体重、病程等临床资料,测定空腹血糖(FBG)、糖化血红蛋白(Hb Alc)、血清chemerin、TNF-α及IL-6水平。另取30名健康人群作为对照组。结果糖尿病患者三组间年龄、身高、体重、病程、糖化血红蛋白无统计学差异。糖尿病骨质疏松组血清chemerin水平高于骨量减少组和骨量正常组(89.7±12.2μg/L;72.5±13.6;65.1±14.9μg/L),差异均有统计学意义(P<0.05)。2型糖尿病合并骨质疏松症组血清炎症因子TNF-α和IL-6水平均高于无骨质疏松症组(P<0.05)。相关性分析结果显示:血清chemerin水平与TNF-α、BMI、Hb A1c、病程相关(r1=0.52;r2=0.76;r3=0.63;r4=0.59;P均<0.05),与腰椎骨密度呈负相关(r=-0.67,P<0.05)。结论chemerin可能通过调控炎症反应参与骨代谢过程,从而促进骨质疏松的发生。
【Abstract】 Objective To explore the relationship between serum chemerin level and bone mineral density( BMD) in type 2diabetic patients. Methods The inpatients with type 2 diabetes were chosen from January 2012 to December 2014 in the Department of Endocrinology of our Hospital. BMD were measured with dual energy X-ray absorptiometry. Patients were divided into type 2 diabetic normal bone mass group( n = 42),type 2 diabetic osteopenia group( n = 50),and type 2 diabetic osteoporosis group( n = 58),according to the WHO diagnostic criteria of osteoporosis. Height,weight,duration of diabetes,and other clinical data were recorded. Fasting blood glucose( FBG),glycosylated hemoglobin( HbA1 c),serum chemerin,TNF-α,and interleukin-6( IL-6) were measured. Another 30 cases of normal people were also included in the study as the control group. Results No significant difference of age and sex,height,weight,duration of diabetes,and HbA1 c was found in the type 2 diabetic patients among the three groups. Serum chemerin level in type 2 diabetic osteoporosis group was significantly higher than that in type 2diabetic osteopenia and normal bone mass group( 89. 7 ± 12. 2 μg /L,72. 5 ± 13. 6,and 65. 1 ± 14. 9 μg /L,P < 0. 05). The TNF-α and IL-6 levels in diabetic osteoporosis group were significantly higher than those in type 2 diabetic normal bone mass group( P <0. 05). Correlation analysis showed that: serum chemerin level was positively correlated with TNF-α,BMI,HbA1 c,and duration of diabetes( r1 = 0. 52,r2 = 0. 76,r3 = 0. 63,r4 = 0. 59,P < 0. 05),but was negatively correlated with BMD of L1-4( r =- 0. 67,P < 0. 05). Conclusion Chemerin may be involved in the pathogenesis of diabetic osteoporosis by regulating inflammatory reaction,therefore promotes the occurrence of osteoporosis.
