碳青霉烯类抗菌药物用药频度与常见革兰阴性菌耐药率的相关性分析

【摘要】 目的 分析碳青霉烯类抗菌药物的耐药情况及用药频度与常见革兰阴性菌耐药率的相关性,为临床合理用药提供依据。方法 回顾性调查2010—2015年清华大学附属北京市垂杨柳医院住院病房送检的微生物标本中分离出的12 092株革兰阴性菌(鲍曼不动杆菌2 545株、大肠埃希菌3 528株、肺炎克雷伯菌3 056株、铜绿假单胞菌2 963株)对碳青霉烯类药物(亚胺培南/西司他丁、美罗培南)的耐药率以及碳青霉烯类药物的用药频度,采用Pearson检验分析细菌耐药率与碳青霉烯类药物用药频度的相关性。结果 2010—2015年本院亚胺培南/西司他丁的用药频度(201.75、331.25、312.25、554.75、585.75、703.80)与鲍曼不动杆菌(74.51%、77.82%、77.23%、84.81%、87.62%、87.80%)、肺炎克雷伯菌(2.07%、6.33%、6.04%、8.11%、9.10%、24.47%)、铜绿假单胞菌(19.75%、22.23%、23.43%、29.88%、27.64%、31.28%)对其的耐药率呈明显正相关(r=0.982,P=0.000;r=0.833,P=0.040;r=0.971,P=0.001)。结论碳青霉烯类药物亚胺培南/西司他丁用药频度与部分革兰阴性菌对其耐药率存在相关性。

【Abstract】 Objective To analyze the correlation between defined daily dose sum(DDDs) of carbopenems and antibiotic resistant rate of Gram-negative bacteria.Methods Totally 12 092 stains of Gram-negative bacteria were isolated from microbiological specimen of in-patients in Beijing Chuiyangliu Hospital Affiliated to Tsinghua University from 2010 to 2015,including acinetobacter baumannii,escherichia coli,Klebsiella pneumoniae and pseudomonas aeruginosa.Resistant rates of bacteria to carbopenems(imipenem/cilastatin and meropenem) and the DDDs of carbopenems were analyzed.Results Pearson analysis showed that the DDDs of imipenem/cilastatin from 2010 to 2015(201.75,331.25,312.25,554.75,585.75,703.80) was positively correlated with the resistance rate of acinetobacter baumannii(74.51%,77.82%,77.23%,84.81%,87.62%,87.80%)(r=0.982,P<0.001),Klebsiella pneumoniae(2.07%,6.33%,6.04%,8.11%,9.10%,24.47%)(r=0.833,P=0.040),pseudomonas aeruginosa(19.75%,22.23%,23.43%,29.88%,27.64%,31.28%)(r=0.971,P=0.001).Conclusion There is a correlation between the DDDs of imipenem/cilastatin and the antibiotic resistance of Gram-negative bacteria.
