微创手术治疗先天性心脏病的早期临床效果分析

【摘要】 目的 分析微创手术治疗先天性心脏病的早期临床效果。方法 回顾性分析首都医科大学附属北京安贞医院2011年4月至2014年12月采用微创手术治疗的116例先天性心脏病患者(微创组)的临床资料,并与116例传统正中开胸手术治疗的先天性心脏病患者(正中开胸组)资料做对比性研究。结果 微创组与正中开胸组手术时间、体外循环时间、主动脉阻断时间差异无统计学意义(P>0.05)。微创组患者手术输血率、术后机械通气时间、住重症监护病房时间、住院时间、胸部切口长度、术后美观满意度优于正中开胸组,差异均有统计学意义[16.4%(19/116)比48.3%(56/116),(10±3)h比(16±5)h,(16±8)h比(27±9)h,(5.2±2.2)d比(9.1±1.7)d,(5.3±2.1)cm比(19.6±2.6)cm,76.7%(89/116)比35.3%(41/116),均P<0.05]。2组患者住院病死率、术后并发症发生率差异无统计学意义(P>0.05)。结论 微创手术治疗先天性心脏病创伤小、美观,术后恢复快。

【Abstract】 Objective To assess the early-term effect of minimal invasive surgery in treating congenital cardiac disease.Methods Data of 232 adult patients with congenital cardiac disease who underwent minimal invasive surgery(minimal invasive group,116 cases) and conventional surgery with midline sternotomy(sternotomy group,116 cases) from June 2011 to October 2014 were reviewed.Results The duration of operation time,cardiopulmonary bypass time and cross-clamp time were not significantly different between groups(P > 0.05).The rate of blood transfusion,the duration of mechanical ventilation time,intensive care unit stay time and hospitalization stay time,the length of operative incision,the satisfaction degree of incision appearance in minimal invasive group were significantly better than those in sternotomy group[16.4%(19/116) vs 48.3%(56/116),(10 ±3) h vs(16 ±5) h,(16±8) h vs(27 ±9) h,(5.2 ±2.2) d vs(9.1 ±1.7) d,(5.3 ±2.1) cm vs(19.6 ±2.6) cm,76.7%(89/116) vs 35.3%(41/116)](allP<0.05).The mortality during hospitalization period and incidence of postoperative complications were not significantly different between groups(P > 0.05).Conclusion Minimal invasive surgery is invasive,with better appearance incision and quicker recovery in treating adult congenital cardiac disease compared with conventional surgery.
