【摘要】 目的探讨泮托拉唑对急性重型颅脑损伤并发应激性溃疡的预防作用。方法选取2011年1月~2015年12月我院收治的急性重型颅脑损伤患者162例,随机分为西咪替丁组和泮托拉唑组,各81例,两组患者均给予吸氧、脱水降颅压、控制血压、营养脑细胞、防治感染、维持水电解质平衡等支持治疗,需要留置胃管患者,予持续胃肠减压。西咪替丁组在此基础上,加以西咪替丁预防应激性溃疡,泮托拉唑组在此基础上,加以泮托拉唑预防应激性溃疡。比较两组治疗后胃酸p H值的变化和应激性溃疡的发生率。结果用药后西咪替丁组和泮托拉唑组胃液p H值均较入院时显著上升,差异有统计学意义(P<0.05),并且泮托拉唑组胃液p H值在进食前后较西咪替丁组更为稳定,泮托拉唑组应激性溃疡的发生率较西咪替丁组显著降低(P<0.05)。结论泮托拉唑能够持久抑制胃酸分泌,升高胃酸PH值,保护胃黏膜,可以预防急性重型颅脑损伤并发应激性溃疡的发生率。更多还原

【Abstract】 Objective To explore the preventive effect of Pantoprazole injury complicated with stress ulcer in patients with acute severe traumatic brain.Methods 162 patients with acute severe craniocerebral injury form January 2011 to December 2015 in our hospital were selected and divided into Cimetidine group and Pantoprazole group,81 cases in each group.Two groups of patients were given oxygen,dehydration of intracraninal pressure,control of blood pressure,nutrishing brain cell,prevention of infection maintain water and electrolyte balance and other support treatment,it is necessary to patients with indwelling gastric tube,continuous gastrointestinal decompression.The Cimetidine group was given cimetidine in prevention of stress ulcer on the basis of routine treatment,the Pantoprazole group was given Pantoprazole with the prevention of stress ulcer on the basis of routine treatment.The changes of gastric acid p H and the incidence of stress ulcer were analyzed and compared between the two groups.Results After treatment of Cimetidine group and Pantoprazole group were significantly compared with admission,the p H value of gastric juice increased,the difference was significant(P<0.05),and the Pantoprazole group in the p H value of gastric juice before and after eating was more stable than that of Cimetidine group.Pantoprazole group of stress ulcer incidence compared with Cimetidine group decreased significantly(P<0.05).Conclusion Pantoprazole sustained inhibition of gastric acid secretion,increased acid p H,protect gastric mucosa,the incidence rate of injury can prevent stress ulcer complicated with acute severe traumatic brain.更多还原
