踝关节骨折X线与MRI诊断下胫腓联合损伤的临床研究

【摘要】 目的探讨踝关节骨折X线与MRI在下胫腓联合损伤诊断中的应用价值。方法选取2013年11月~2015年11月本院收治的56例行Lauge-Hansen分型及Denis-Weber分型的踝关节骨折患者作为研究对象,将骨折分型与X线测量有机结合,参照MRI对下胫腓联合损伤的完整性进行评估,分析踝关节骨折X线与MRI在下胫腓联合损伤诊断中的敏感度、特异度及相关性。结果按照MRI诊断,X线测量与Weber分型结合,下胫腓前后韧带损伤的敏感度为49%,特异度为100%;LH分型的敏感度为93%,特异度为94%,韧带损伤情况无法由2种分型判断,但是,在损伤判断方面,LH分型与MRI检查结果存有很多共性。结论急性下胫腓联合损伤尽早得到确诊是治疗的关键和首要前提,可结合LH分型与X线来判断踝关节患者是否发生下胫腓联合韧带损伤,而具体诊断下胫腓韧带损伤的程度则推荐应用MRI来进行详细诊断。
【Abstract】 Objective To explore the application value of ankle fracture X-ray and MRI in the diagnosis of lower tibia and fibula combined injury.Methods 56 patients with ankle fracture conducted Lauge-Hansen type and Denis-Weber type from November 2013 to November 2015 in our hospital were selected as study objects.Fracture type and X-ray measurement were organic combined.The integrity of lower tibia and fibula combined injury was evaluated according to MRI,the sensitivity,specificity and correlation of X-ray and MRI in the diagnosis of lower tibia and fibula combined injury was analyzed.Results According to MRI diagnosis,X-ray measurement and Weber type combination,the sensitivity was 49%,and the specificity was 100%;the sensitivity of LH type was 93%,and the specificity of LH type was 94%,and ligament injury could not be judged by 2 types,but in the field of damage judgment,LH type had many generality with MRI inspection result.Conclusion Early diagnosis and treatment of acute lower tibia and fibula combined injury is the key and prerequisite for the treatment of patients,combination LH type and X-ray can judge whether ankle patients with lower tibia and fibula combined injury,while the specific diagnosis of the injury degree of the tibia and fibula ligament,MRI is recommended to use for making a detailed diagnosis.
