【摘要】 目的探讨大隐静脉高位结扎+主干顺行抽剥+分支曲张静脉泡沫硬化剂注射疗法治疗下肢大隐静脉曲张的效果。方法将我院2014年6月~2015年3月收治的40例(40条患肢,均单侧患病)临床分期C4~C6的下肢静脉曲张患者随机抽签分成2组。对照组采用传统手术,即大隐静脉高位结扎+主干逆行抽剥+分支曲张静脉多点抽剥术;实验组采用大隐静脉高位结扎+主干顺行抽剥+分支曲张静脉泡沫硬化剂注射术。比较两组患者的切口数量、切口总长度、术中出血量、手术时间、住院时间、溃疡愈合时间、术后并发症发生情况。结果两组患者住院时间、溃疡愈合时间比较,差异无统计学意义(P>0.05);实验组切口数量多于对照组,切口总长度、手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。实验组并发症发生率为20%,对照组为30%,两组比较差异无统计学意义(χ2=1.549,P>0.05)。结论大隐静脉高位结扎+主干顺行抽剥+分支曲张静脉泡沫硬化剂注射术治疗下肢静脉曲张安全有效、切口少且小、损伤小、并发症少、美观微创,优于传统手术,可作为大隐静脉曲张治疗的首选术式。更多还原

【Abstract】 Objective To investigate the effect of great saphenous vein high ligation of main +direct extraction stripping +branch varicose vein foam sclerosing agent injection therapy in the treatment of lower limb varicosis of great saphenous vein. Methods 40 patients with clinical stage C4-C6 varices of lower extremity in our hospital from June2014 to March 2015 were divided into two groups randomisely.The control group used the traditional surgical,namely high ligation of great saphenous vein stripping+trunk retrograde branch varicose vein stripping operation point.The experimental group used great saphenous vein high ligation of main+direct extraction stripping+branch varicose vein foam sclerosing agent injection therapy.The incision number,total length of incision,bleeding volume,operation time,length of stay,healing time and postoperative complications were compared between the two groups. Results The hospital time,ulcer healing time between the two groups had no statistical differenc(P>0.05).The number of incision in the experimental group was more than that in the control group,the total length of the incision,operation time was shorter than that of the control group respectively,the amount of blood loss was less than that of the control group,the difference was statistically significant(P<0.05).The complication incidence of the experimental group was 20%,30% in the control group,there had no statistically significant difference between the two groups(χ~2=1.549,P>0.05). Conclusion Great saphenous vein high ligation of main+direct extraction stripping+branch varicose vein foam sclerosing agent injection therapy is a safe and feasible procedure with minimal invasion,better cosmetic result,quicker recovery,shorter hospitalization time.It is worth using widely.更多还原
